我院门诊西药处方点评及合理性评价分析

摘　　要：目的了解我院门诊西药处方常见问题,规范处方管理,促进临床合理用药。方法随机抽取医院2014年1月-6月1800张门诊处方,根据《处方管理办法》、《医院处方点评管理规范(试行)》以及《抗菌药物临床应用指导原则》进行点评、统计和分析。结果我院门诊处方平均每张用药品种数为2.23种,抗菌药物使用率平均为31.00%,注射剂使用率平均为28.83%。在1800张门诊处方中,不合理处方344张,不合格率为19.11%。其中,不规范处方152张,占不合格处方的44.19%;用药不适宜149张,占不合格处方的43.31%;超常处方43张,占不合格处方12.50%。结论医院门诊处方基本合理,但尚存在较多问题,处方合格率有待进一步提高,抗菌药物存在不合理使用现象,医师和药师需要共同学习,仍需加强对处方的管理、审核及干预以进一步提高处方质量。Objective To investigate the common problems in our hospitals outpatient prescriptions, standardize the management of prescription and promote rational clinical drug use. Methods 1800 outpatient prescriptions from January 2014 to June 2014 were randomly selected. Comments, counting and analysis were conducted based on documents such as Administrative Rules on Prescriptions, Standard Management of Hospital Prescription Comments （ Draft ） , and Guiding Principles of Clinical Use of Antibaeterials. Results For an average outpatient prescription,2.23 drugs were administered. The antibacterials use accounted for 31% and the injections use was 28.83 %. Of the 1800 prescriptions,344 were singled out as irrational, with an unqualified rate of 19.11%. 152 prescriptions were identified as non-standard, accounting for 44.19% of the irrational 344. 149 were taken as inappropriate, the rate being 43.31%. 43 of the non-standard prescriptions were rated as extraordinary,with a rate of 12.5%. Conclusion The outpatient prescriptions of our hospital are basically reasonable,though some problems are still to be settled and the qualified rate is to be raised. Irrational use of antibacterials are still prevalent to some extent. Doctors and pharmacists should further their professional studies, reinforce drug administration, examination and intervention so as to improve the quality of prescriptions.
