腭帆成形术联合硬腭截短软腭前移术治疗Ⅱ型中重度OSAHS的疗效分析

摘　　要：目的:探讨腭帆成形术(VPP)联合硬腭截短软腭前移术治疗Ⅱ型中重度OSAHS的疗效。方法:以32例经PSG确诊为中重度OSAHS的患者为研究对象,术前阻塞平面定位于上气道软腭平面(Ⅱ型),行VPP联合硬腭截短软腭前移术,对比手术前后Epworth嗜睡量表(ESS)评分、AHI和最低血氧饱和度(LSaO2)结果。结果:32例患者手术有效率为78.12%(25/32),其中治愈3例(9.38%);手术前后患者的ESS评分、AHI、LSaO2比较均差异有统计学意义(均P<0.05);AHI降幅<50%的7例患者,其ESS评分和LSaO2手术前后比较差异有统计学意义(P<0.05)。结论:VPP联合硬腭截短软腭前移术可明显提高OSAHS的疗效。Objective：To explore the curative effect of velopharyngoplasty（VPP）combined with hard palate shorting and soft palate forward in the treatment of typeⅡ moderately severe obstructive sleep apnea hypopnea syndrome（OSAHS）.Method：Thirty-two patients with OSAHS with and the obstructed surface located in soft palate were subjected to VPP combined with hard palate shorting and soft palate forward.The ESS scores,result of PSG included AHI and the lowest oxyhemoglobin saturation before and after operation were compared.Result：The effective rate of VPP was 78.12%this group.The grade of ESS,PSG and the lowest oxyhemoglobin saturation was statistically significant before and after the operation（P0.05）.In the other 7cases whose AHI decrease less than 50%,the grade of ESS,AHI and the lowest oxyhemoglobin saturation was also statistically significant.Conclusion：VPP combined with hard palate shorting and soft palate forward can improve the surgical effective rate obviously for OSAHS patients.
