两种术式治疗翼状胬肉的效果比较

摘　　要：目的：对比两种手术方法治疗原发性翼状胬肉的临床疗效.方法：原发性翼状胬肉90例（112眼）随机分成两组.A组50例（58眼）行翼状胬肉切除联合带蒂结膜瓣转位术;B组40例（54眼）行翼状胬肉切除联合自体角膜缘干细胞移植.比较两组患者手术时间、角膜创面上皮愈合时间及复发率.结果：A组手术时间为（29.09-± 2.70）min,明显短于B组（32.72±2.41）min.B组角膜创面上皮愈合时间明显快于A组,组间比较,差异具有统计学意义（P＜0.05）.术后随诊6～18个月,A组有2眼复发（3.45％）,B组有1眼复发（1.85％）,差异无统计学意义（P＞0.05）.结论：两种术式均能有效阻止胬肉复发.但角膜缘干细胞移植术治疗翼状胬肉具有愈合快、并发症少、舒适性高等优点,是目前最值得推广的治疗原发翼状胬肉手术方式。Objetive To compare the efficacy of two different surgery methods of primary pterygium. Methods 90 patients （112 eyes）were randomly divided into two groups.Group A （50 patients）; pterygium excision with conjunctival flap transposition in 58 eyes;Group B （40 patients）：pterygium excision with corneal limbal stem cell autograft transplantation in 54 eyes.The recurrence rate and corneal epithelial wound healing time were observed respectively. Results The average operative time was （29.09±2.70） min in group A, which was significantly shorter than that of group B （32.72±2.41）min.The corneal epithelial wound healing time of group B faster than group A,the difference was statistically significant （P〈0.05）.All cases were followed up for 6-18 months. Pterygium relapsed in 2 eyes （3.45%）in Group A and in 1 eyes （1.85%） in Group B（P〉0.05）. Conclusion Both corneal limbal stem cell autograft transplantation and conjunctival flap transposition are effective in treating of pterygium.But the former appears to be faster healing,less complications and comfortable than the latter.It is the most worthy of promotion in the treatment of primary pterygium surgery.
