摘  要:目的:通过比较研究绿茶及葡萄糖酸亚铁糖浆(FGS)用于MRCP检查,旨在探索将绿茶作为一种阴性口服对比剂用于MRCP的安全性和可行性。方法:将受检者随机分为绿茶组(n=20)和FGS组(n=20),分别采集对比剂口服前、口服后5min及口服后15min的MRCP图像,对胰胆管系统、胃和十二指肠内信号丢失情况进行图像质量评估,并进行统计学分析。结果:对远端胆总管显示情况,绿茶组口服前后与口服后2组图像间差异具有显著性意义(F=5.309),且口服前与口服后5min、15min其差异具有统计学意义(绿茶组:P=0.008及0.013)。对于胃及十二指肠各段内液体信号的消除程度,2组口服前后评分的差异均具有显著性意义,且口服后5、15min均高于未服组(P<0.05)。口服对比剂5min后,2组仅对肝内胆管评分差异具有统计学意义,F值=3.632。茶叶较FGS组好,其差异具有统计学意义(P<0.05)。结论:绿茶可作为MRCP的口服胃肠道阴性对比剂。MRCP最佳采集图像时间为口服绿茶后5min,绿茶作为口服对比剂用于MRCP检查值得推广。Purpose:Prospective evaluation of the efficacy of green tea as a negative oral contrast agent used in MRCP by comparing the image quality of magnetic resonance cholangiopancreatography (MRCP)using green tea and Ferrous Gluconate Syrup (FGS) as the negative oral contrast agent.Methods:MRCP was performed before and 5 and 15 min after oral agents were used.Patients received green tea (n =20) or FGS (n=20) in a random way.The subjective image quality of the gall bladder (GB),cystic duct (CD),proximal and distal parts of the common bile duct (CBD),intrahepatic ducts (IHD),common hepatic ducts (CHD),main pancreatic duct (MPD) and signal loss of stomach and three different segments of the duodenum were investigated according to Likert scores by two radiologists blinded to information regarding the agent were compared using Friedman test.Results:Regarding visibility of different anatomical parts of the pancreatobiliary tree,the images of green tea group were better visualized in the distal part of CBD (F =5.309),and the quality of 5 min,15 min post-agent images of green tea group were significantly better than before-agent images,respectively (P=0.008 and 0.013).Regarding obliteration of high signal in the stomach and three different parts of the duodenum,images in both groups showed significant disappearance of high signal in Likert scoring systems (P〈0.05).For the subjective images quality of IHD,green tea group were significantly better than FGS group (F=3.632).Conclusion:Following green tea ingestion,the optimum time for MRCP is 5 min.Green tea is a simple and safe negative oral contrast agent which reduces the signal intensity of gastrointestinal tract fluid and provides improved depiction of pancreatobiliary tree during MRCP.
