320排CT下肢动脉造影中对比剂用量的合理选择

摘　　要：目的:探讨320排CT下肢动脉造影中使用低剂量对比剂的可行性。方法:60例临床疑有下肢动脉阻塞性病变的患者按随机分类法分成3个使用不同剂量对比剂组,每组20例,而后行320排螺旋CT下肢动脉CTA检查。每组分别测量髂总动脉、股深动脉及胫后动脉CT值进行比较;并对图像按"优、良、差"作3级评价。结果:三组各相同动脉间CT值差异无统计学意义。各下肢动脉显像在三组病例中均能达到优或良,合计优良率均为100%(20/20),图像评分上3组间统计学差异无显著性意义(χ2=2.846,P=0.154)。结论:320排螺旋CT下肢动脉造影中,应用低剂量对比剂(70ml),采用合理的对比剂注射方法进行下肢血管造影,既能减少对比剂的用量,减轻对比剂对患者身体的危害,同时也可满足临床诊断的要求。Purpose：To investigate the feasibility of using low dose contrast agent for 320-slice CT angiography （CTA） of lower extremity arteries.Methods：CTA of lower extremity arteries were performed in 60 patients suspected with peripheral arterial occlusive disease.They were divided randomly into three groups according to the dosage of contrast agent.CT attenuation values of the common iliac artery,deep femoral artery and posterior tibial artery were measured for each group and the image quality was evaluated as excellent,good,or poor.Results：No significant difference of CT attenuation values was shown among the three groups.The image quality of lower extremity arteries in three groups was well delineated as excellent or good,and there was not significant difference among them （x2=2.846,P=0.154）.Conclusion：It is feasible to use low dose contrast agent （70ml） for 320-slice CTA of lower extremity arteries,at the same time it may reduce potential risk of nephropathy.
