摘 要:目的探讨甲状腺影像报告和数据系统(TI-RADS)分级在甲状腺微小癌(TMC)超声检查中的应用。方法对最大径≤1.0 cm的232例甲状腺结节患者的322个甲状腺结节进行TI-RADS分类,并与病理结果对照分析。结果统计学分析表明,甲状腺良性结节多见于TI RADS 2级、3级、4A级类结节,恶性结节多见于TI-RADS 4A级、4B级、5级结节;两者诊断甲状腺结节良恶性的差异具有统计学意义(χ2=110.10,P<0.01)。其诊断甲状腺结节的灵敏度和特异度分别为74.3%(110/148)和75.9%(132/174)。超声TI-RADS分级诊断的受试者工作特征曲线(ROC)的曲线下面积为0.822。结论 TMC的TI-RADS分级可较好地用于甲状腺结节良恶性的诊断鉴别。Objective To explore the application of Thyroid Imaging Reporting and Data System (TI-RADS) in ultrasound examination of thyroid microcarcinoma (TMC). Methods A total of 232 patients with 322 thyroid nodules with diameter less than or equal to 10 mm were evaluated by using TI-RADS ultrasonographic stratification, and the results were compared with pathological analysis. Results The benign nodules were commonly classified as TI-RADS 2, 3 and 4A, and the malignant nodules were classified as TI-RADS 4A, 4B and 5. The differences were statistically significant (χ2=110.10,P&lt;0.01). The sensitivity and specificity of TI-RADS in diagnosis of benign and malignant thyroid lesions were 74.3% (110/148) and 75.9% (132/174), respectively. The area under receiver operating characteristic curve was 0.822. Conclusion TI-RADS ultra-sonographic stratification of TMC can be well used in differentiation of benign and malignant nodules.
