经尿道前列腺等离子电切术中失血量的相关因素分析

摘　　要：目的探讨经尿道前列腺等离子电切术（transurethral plasma kinetic resection of prostate,PKRP）中影响失血量的可能相关因素。方法回顾分析137例应用PKRP治疗前列腺增生（benign prostate hyperplasia,BPH）患者临床资料,以年龄、病程、糖尿病、高血压、高脂血症、前列腺体积、膀胱结石、反复血尿、留置导尿管、非那雄安、抗血小板药、手术时间、前列腺切除质量、术中并发症和术者手术年资等为自变量,应用多因素逐步回归分析各因素与出血量之间的关系,寻找影响术中失血量的主要因素。结果平均手术时间（87±27）min（45∽118 min）,平均术中失血量（308±37）ml（78∽716 ml）。分析显示前列腺体积、糖尿病、抗血小板药物、术前口服非那雄胺、手术时间、术者手术年资及术中并发症对术中失血量的影响有统计学意义（P〈0.05）,其中手术时间是最主要的影响因素（β＇=0.417）。年龄、病程、高血压、高脂血症、留置尿管、泌尿道感染、血尿、膀胱结石等对术中失血量影响不明显。结论手术时间是经尿道前列腺等离子电切术中失血量的最重要影响因素。Objective To identify factors influencing hemorrhage during transurethral plasma kinetic resection of prostate （PKRP）. Methods The clinical data of 137 patients with benign prostate hyperplasia （BPH） who underwent PKRP were retrospectively analyzed. Various factors were assessed for correlation with total blood loss using stepwise multivariate regression analysis. Results The average operating duration was （87 ± 27 ）min （45 -118 min）, the average blood loss was （308 ±37）ml （78 -716 ml）. Stepwise multivariate regression analysis showed that volume of prostate, diabetes mellitus, preoperative finasteride, anti-platelet drug, operation time, seniority of surgeon and complications during operation were significantly correlated with intraoperative blood loss （P 〈0.05）, and the operating duration was the most influencing factor on blood loss （β＇ = 0. 417 ）. Age, course of disease, hypertension,hyperlipidemia, urinary infection, indwelling catheter, hematuria and bladder calculi were not correlated with blood loss （P 〉 0.05 ）. Conclusion Many factors would influence intraoperative blood loss during transurethral plasma kinetic resection of prostate, particularly the operating time.
