基于肺小叶中心纤维化CT表现筛查胃食道返流病的初步研究

摘　　要：目的：是描述慢性肺微吸入的高分辨率CT的肺纤维化表现，并通过CT的肺纤维化表现筛查GERD。方法：连续抽取食道胃镜检查证实的GERD病例的肺CT检查资料86例。通过三位具有经验的影像科医师分别读片及共同读片，最后筛选出24例资料。肺CT扫描范围从肺尖到肺底，然后0．625～1．25mm薄层骨算法重建。对所有病人CT图像进行观察，以IPIF CT表现为诊断标准提取影像表现。结果：24例CT的肺纤维化表现是：（1）班片状磨玻璃影例24（100％），其中局部中心分布22例（92％），合并周边性分布17例（70％）。（2）胸膜下蜂窝影9例（37％），其中合并间隔线影7例（3）小叶中心结节影7例（29％），其中合并小灶样实变5例。（4）牵拉性细支气管扩张15例（62％）。（5）中下段食道扩张17例（70％）。结论：GERD伴有的肺纤维化CT表现主要表现为：局部中心分布的班片状磨玻璃样影以及周边分布班片状磨玻璃影、胸膜下蜂窝影、小叶中心结节影伴食道扩张、细支气管扩张，如具有这些CT表现高度提示GERD的可能。Objective：The aim of the study was to describe the high-resolution computed tomography（ CT） manifestations of chron-ic pulmonary microaspiration on the diagnosis of GERD.Materials and Methods： We reviewed the CT findings in 86 consecutive pa-tients with GERD of chronic pulmonary microaspiration proved by an endoscopy.All patients had volumetric CT of the chest reconstruc-ted using thin sections（0.625to1.25mm） at the time of diagnosis.The CT scans were interpreted by 3 radiologists who reached a final decision.We evaluated 24 patients who met criteria for the diagnosis of IPIF.Results：（1）All 24 patients （100%）had patchy ground-glass opacities that involved centrilobular in 22 patients （92%）and with peripheral 17 patients（70%）.Other common findings in-cluded （2） honeycomb（n=9）,（37%） （3） centrilobular nodules（n=7）（29%）,（4） bronchiectasis（n=15）（62%）,（5） esopha- 〈br〉 geal dilatation（n=17）（70%）.Conclusions：HRCT manifestations of GERD with chronic pulmonary microaspiration consist mainly of centrilobular patchy ground-glass opacities.Centrilobular nodules, honeycomb, bronchiectasis, esophageal dilatation are seen in the majority of cases.
