经腹部联合经阴道超声在妇科急腹症中的临床价值

摘　　要：目的探讨经腹部超声(TAS)联合经阴道超声(TVS)诊断在妇科急腹症中的临床价值。方法采用数字表法抽选2010年3月至2013年3月武警总医院收治的妇科急腹症患者167例,并将TAS联合TVS诊断结果以及单独应用TAS诊断结果与病理诊断进行对比,判断其诊断的符合率。结果 167例患者均予以病理学检查确诊,单独进行腹部超声检查的临床病理诊断符合率为83.6%(138/167),漏诊15例,误诊14例;167例患者在TAS检查基础上联合TVS检查,与病理诊断结果的符合率为95.2%(159/167),漏诊5例,误诊3例,两种检测方法的病理诊断符合率比较,差异有统计学意义(P<0.05)。结论 TAS联合TVS能够提高超声诊断的有效率,为妇科急腹症的临床诊断提供技术基础。Objective To explore the clinical value of joint detection of transabdominal ultrasound （TAS） and transvaginal ultrasound （TVS） in the diagnosis of gynecological acute abdomen. Methods A totalof 167 cases of patients with gynecological acute abdomen admitted in General Hospital of Chinese People＇s Armed Police Forces from Mar. 2010 to Mar. 2013 were selected by random number table, diagnostic results detected by TAS combined with TVS and by single TAS were compared with pathological diagnostic results to determine their diagnostic coincidence rates. Results All of the 167 cases were diagnosed by pathologieal examination, compared with pathological diagnostic result, diagnostic coincidence rate of single TAS detection was 83.6% （ 138/167 ） , with 15 cases of missed diagnosis, and 14 cases of misdiagnosis; diagnostic coincidence rate of joint detection of TAS and TVS was 95.2% （159/167） ,with 5 cases of missed diagnosis, and 3 cases of misdiagnosis, difference of the diagnostic coincidence rates of the two detection methods was statistically significant （ P 〈 0.05 ）. Conclusion Joint detection of TAS and TVS can improve the efficiency of ultrasonic diagnosis, and provide technical foundation for the clinical diagnosis of gynecological acute abdomen.
