儿童细菌性肺炎血清降钙素原、白细胞介素、肿瘤坏死因子及全血超敏C-反应蛋白的检测意义

摘　　要：目的:分析细菌性支气管肺炎儿童血清降钙素原(PCT)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)和全血超敏C-反应蛋白(hs-CRP)等感染标志物的变化,探讨其临床意义。方法:对47例细菌性支气管肺炎儿童(观察组)和45例健康儿童(对照组)采用双抗体夹心酶联免疫吸附法测定血清PCT、IL-6、IL-8、TNF-α水平,采用速率散射比浊法测定全血hs-CRP水平,并进行比较。结果:观察组治疗前血清PCT、IL-6、IL-8、TNF-α和hs-CRP水平明显增高,与对照组比较差异有统计学意义(P<0.05);治疗后血清PCT、TNF-α和全血hs-CRP水平恢复正常(P>0.05),血清IL-6、IL-8水平虽降低,但与对照组比较仍然偏高,差异有统计学意义(P<0.05)。结论:血清PCT、IL-6、IL-8、TNF-α和全血hs-CRP等感染标志物对儿童细菌性支气管肺炎诊断和治疗有一定意义,且有助于观察疗效和随访预后,尤以血清PCT检测重要,值得临床推广和进一步研究。Objective： To explore the clinical analysis of determination of serum procalcitonin （PCT）, interleukin-6 （IL-6）, interlenkin-8 （IL-8） , tumor necrosis factor-α （TNF-α） and hyper-sensitive C-reaction protein （hs-CRP） levels in pediatrics patients with bacterial bronchopneumonia. Methods： The serum PCT, IL-6, IL-8, TNF-α levels were analyzed in 47 pediatrics patients （observation group） with bacterial bronchopneumonia and 45 healthy children （control group） by double antibody sandwich enzymelinked immunosorbent assay and the hs-CRP levels were analyzed by rate nephelometry as well as done study compared. Results： Before treatment, the PCT, IL-6, IL-8, TNF-α and hs-CRP levels in observation group were significantly higher than those of control group （P〈0.05）. After treatment, the serum PCT, TNF-α and hs-CRP levels in observation group were returned to normal （P〉0.05）, the serum IL-6 and IL-8 levels were decreased, but it still significantly higher than those of control group （P〈O. 05 ）. Conclusion： The determination of PCT, IL-6, IL-8, TNF-α and hs-CRP levels were valuable index for diagnosis and therapy following in pediatrics patients with bacterial bronchopneumonia and the determination of serum PCT levels were most important.
