摘　　要：目的:研究完全水解蛋白配方奶(CHPF)对早产儿生长发育、喂养安全性的影响。方法:采用前瞻性随机对照研究方法,将我院NICU收治的早产儿40例分为CHPF组及标准早产儿配方奶组(SPF组),比较两组的体格发育指标、喂养不耐受率、达完全肠道喂养时间、静脉营养使用时间、恢复出生体质量时间。结果:与SPF组比较,CHPF组喂养不耐受发生率降低(6.25%vs 33.33%),每天体质量增长[(18.06±1.84)g vs(13.58±1.74)g]、每周身长增长[(1.07±0.14)cm vs(0.89±0.18)cm]、每周头围增长[(0.78±0.12)cm vs(0.52±0.09)cm]加快,达全胃肠喂养时间[(16.9±4.4)d vs(21.1±5.0)d]、静脉营养时间[(11.6±2.1)d vs(14.1±2.0)d]及恢复出生体质量时间[(7.4±0.8)d vs(9.3±1.8)d]缩短,差异均有统计学意义(P均<0.05)。结论:CHPF能够促进胃排空,改善早产儿喂养不耐受,并使其更早达到全胃肠道喂养,从而促进早产儿生长发育;CHPF可用于早产儿开奶。Objective： To investigate the effect of completely hydrolyzed protein formula （CHPF） milk on the growth and the safety in premature infant. Methods： A prospective randomized study was performed, 40 premature infants who treated in NICU were randomly divided into CHPF group （n = 16） and standard premature infant formula milk group （SPF group） （n =24）. Compared the physical development index, the incidence of feeding intolerance, the time of intravenous nutrition and regaining the birth weight between two groups. Results： Compared winh SPF group, the incidence of feeding intolerance in CHPF group was much lower （6.25% vs 33.33% ）, the growth velocity of body weight in one day [ （ 18.06±1.84） g/d vs （ 13.58± 1.74 ） g/d ], body length in one day [ （ 1.07 ±0.14） cm/w vs （0.89±0.18） cm/w] , head circumference in one week [ （0.78±0.12） cm/w vs （0.52±0.09） cm/w] in CHPF group were increased more quickly. For the time to achieve full enteral feeding [ （16.9±4.4） d vs （21.1 ±5.0） d], regain the birth weight [ （7.4±0.8） d vs （ 9.3±1.8 ） d ] and the duration of parenteral nutrition [ （ 11.6±2.1 ） d vs （ 14.1 ±2.0 ） d ] in CHPF group was shorter than SPF group. Conclusion： CHPF can improve gastric emptying and feeding tolerance, and enable a more rapid establishment of full enteral feeding in preterm infant compared with SPF, so it can be used as the early opening of milk.
