摘　　要：目的探讨丙泊酚靶控输注用于人工流产术镇痛的半数有效效应室浓度(EC50)。方法 46例无痛人工流产术患者依据阴道分娩史分为P组(有阴道分娩史,22例)和N组(无阴道分娩史,24例),采用丙泊酚靶控输注。在麻醉诱导期和维持时段选择不同丙泊酚效应室浓度输注,计算两组麻醉诱导期和维持时段的丙泊酚EC50及95%可信区间(CI)。结果 P组麻醉诱导期的丙泊酚EC50和95%CI分别为3.88μg/ml和(3.70-4.07)μg/ml,与N组的3.91μg/ml和(3.66-4.16)μg/ml相仿(P>0.05)。P组麻醉维持时段丙泊酚的EC50和95%CI分别为5.92μg/ml和(5.55-6.31)μg/ml,低于N组的6.40μg/ml和(6.14-6.66)μg/ml(P<0.05)。结论人工流产术麻醉诱导期有、无阴道分娩史患者EC50接近,而麻醉维持时段无阴道分娩史患者需要较高水平的丙泊酚EC50。Objective To investigate the median effective concentration（EC50）of propofol of analgesia by target controlled infusion（TCI）in the patients undergoing artificial abortion.Methods Forty-six patients undergoing pain-free artificial abortion were divided into two groups of P（with the history of vaginal delivery,22cases）and N（without the history of vaginal delivery,24cases）.During the period of induction and maintenance,both groups were given propofol TCI in different EC50 and the EC50 and 95% confidence interval（CI）of propofol TCI were calculated.Results During induction,the EC50 and 95% CI of propofol TCI in group P were 3.88μg/ml and（3.70-4.07）μg/ml,which were similar to 3.91μg/ml and（3.66-4.16）μg/ml in group N（P〉0.05）.During the period of maintenance,the EC50 and 95% CI of propofol TCI in group P were 5.92μg/ml and（5.55-6.31）μg/ml,which were lower than 6.40μg/ml and（6.14-6.66）μg/ml in group N（P〈0.05）.Conclusion The EC50 for analgesia with propofol TCI during induction of pain-free artificial abortion is similar in the patients with and without history of vaginal delivery,which is more during the period of maintenance in the patients without history of vaginal delivery.
