摘  要:心肌缺血再灌注损伤(MIRI)严重危害人和动物的生命健康,如何防治是医学难点。麻醉镇静药在手术和检查中不可缺少,其对MIRI的影响是研究的热点。右美托咪啶(Dex)被誉为21世纪镇静剂,应用前景广阔,研究发现其具有抗肺脏损伤、抗肾脏损伤等器官保护作用,以及心脏保护作用,还可能涉及抗缺血再灌注损伤、抗心律失常等方面。文章探讨了自由基、细胞内钙超载、心肌细胞凋亡等影响MIRI的因素,分析了MIRI P13K-Akt-e NOS和JAK/STAT信号转导通路的影响;从器官、细胞和分子水平探讨右美托咪啶对MIRI的作用机制,为防治MIRI提供新思路,也为合理应用右美托咪啶提供参考。Myocardial ischemia reperfusion injury (MIRI) seriously endangers the life and health of humans and animals, it is a medical diffi- culty that how to prevent. Narcotic sedatives play an indispensable role in surgery and examination, their effects on the MIRI are the focus of re- search. Dexmedetomidine (Dex) is known as the sedative for the 21 st century, and has a broad application foreground. The studies find that Dex has protective effects against lung injury, and kedney injury, and it has cardioprotective effect, may be also involved in anti - ischemic reperfusion injury, antiarrhythmic, and so on. This paper investigates the factors that affect MIRI, including radicals, intraceliular calcium o- verload, cardiac myocyte apoptosis, and so on. It analyzes the effect of MIRI on PI3K - Akt - eNOS and JAK/STAT signal transduction path- way, and also explores the mechanism of action of Dex on MIRI from the organ, cellular and molecular levels. It provides new ideas for the pre- vention and treatment of MIRI and also provides a reference for the rational application of Dex.
