多层螺旋CT在胃结肠癌术前诊断及术中切除中的应用价值

摘　　要：目的探讨多层螺旋CT检查对胃结肠癌术前评估的应用价值。方法回顾性分析我院经病理证实的30例胃癌及66例结肠癌患者的临床及CT资料,分析病变的数目、大小、形态、密度、边界、强化方式,应用多平面重建观察邻近组织的浸润情况。结果 MSCT对胃癌T期及N期诊断的符合率分别为86.20%及79.30%,相应一致性检验Kappa值分别为0.772及0.561。MSCT对结肠癌T期、N期及TNM分期诊断的一致性检验Kappa值分别为0.847、0.718、0.819。结论 MSCT多平面重建技术可多角度、全方位、任意平面地观察病变的特点及其与周围组织器官之间的关系,可为胃结肠癌术前评估作出准确判断。Objective To explore the value of clinical diagnosis and preoperative assessment of multi-slice spiral com- puted tomography （MSCT） in colon and stomach cancer. Methods Clinical data and CT findings of 30 patients with stomach cancer and 66 patients with colon cancer, confirmed by operation and pathologic examination, were retrospectively analyzed. The size, shape, density, boundary and strengthen of the mass, and infiltration of adjacent tissue with muhiplanar reconstruction were observed. Results The diagnosis accuracy of MSCT for stage T of gastric cancer was 86.20%, and that of the Kappa value of consistency check was 0.772, 0.561. The diagnosis accuracy of MSCT for stage N of gastric cancer was 79.30%, and that of the Kappa value of consistency check was 0.561. The Kappa value of consistency check for stage T, N and TNM of gastric carcinoma was 0.847, 0.718 and 0.819, respectively. Conclusion Multiplanar reconstruction of MSCT can observe the relationship between the lesions and surrounding tissues and organs in all directions, multi-angle and any plane, and can make accurate preoperative assessment for colon and stomach cancer.
