重症感染休克患者血小板参数变化在治疗中的临床意义

摘　　要：目的探讨血小板参数值对判断重症感染休克患者病情和愈后程度的意义,以判断病情变化趋势,便于合理治疗。方法回顾性分析2013年1月-2014年3月101例患者临床资料,根据愈后分为死亡组62例与治愈组39例,记录入院4d内和出院前3d血小板计数、平均容积、分布宽度、压积及大血小板比率、最大聚集率等数值。结果死亡组患者的血小板容积、大血小板比率和分布宽度均呈上升趋势,血小板计数、压积数值、最大聚集率则呈逐渐下降趋势;治愈组患者相应参数呈相反趋势。结论感染性休克死亡组与治愈组的患者血小板参数有不同的变化趋势,这些变化在临床上可提示患者病情的状况和愈后程度,值得在临床推广使用。OBJECTIVE To explore the significance of platelet parameters in determination of the illness condition and prognosis of the patients with severe septic shock so as to understand the trend of illness condition and conduct reasonable therapy .METHODS The clinical data of 101 patients who were treated in the hospital from Jan 2013 to Mar 2014 were retrospectively analyzed ,then the patients were divided into the death group with 62 cases and the cured group with 39 cases according to the prognosis ,and the platelet counts ,average volume ,distribution width , hematocrit ,ratio of large platelets ,and rate of maximum platelet aggregation were recorded within 4 days of en‐rollment to the hospital or at three days before discharge .RESULTS The platelet volume ,ratio of large platelet , and distribution width of the patients in the death group showed an upward trend ,while the platelet counts ,hema‐tocrit value ,and rate of maximum platelet aggregation presented a downward trend ;the relevant parameters of the patients in the cured group showed the opposite trend .CONCLUSION The septic shock patients in the death group and the cured group vary in the change trends of the platelet parameters ,which may indicates the illness condition and prognosis of the patients ,and it is worthy to be promoted in the hospital .
