综合挑战测试包与自制测试包在医院压力蒸汽灭菌应用研究

摘　　要：目的过观察3M公司提供的Attest 41382型综合挑战测试包与自制生物监测包在压力蒸汽灭菌器中的灭菌对比,规范高压蒸汽灭菌过程监控的流程、测试装置,确保灭菌效果,降低成本支出,提高工作效率,降低医院感染。方法将挑战包和自制包同时放入高压灭菌锅最难灭菌的底层、近门处排气口上方灭菌,调节不同的参数,分别为常规灭菌参数和模拟失败临界灭菌参数,对比两种灭菌参数下两种测试包的化学指示卡合格率、生物监测合格率及成本和影响因素。结果在常规灭菌参数下,挑战包和自制包灭菌后的包外指示带均变色合格、爬行卡及生物培养合格率均为100.0%;在模拟失败临界灭菌参数下,包外指示带爬行卡合格率为挑战包83.3%,自制包灭菌后为90.0%;生物培养合格率挑战包为63.3%,自制包为80.0%,成品综合挑战测试包和自制测试包在同种灭菌参数条件下灭菌结果差异无统计学意义。结论根据试验证明综合挑战测试包在医院压力蒸汽灭菌应用中符合规范,自制测试包在医院压力蒸汽灭菌中的准确性的影响因素大于成品综合挑战测试包。OBJECTIVE To observe the performances of the 3M Attest 41382 comprehensive challenge test pack and the hand made test pack in the pressure steam sterilization ,standardize the procedure of monitoring of high pres‐sure steam sterilization ,and test the device so as to ensure the sterilization effect ,reduce the cost ,raise work effi‐ciency ,and reduce the incidence of nosocomial infections .METHODS The challenge packs and hand‐made test packs were together placed into the bottom of the high pressure sterilization and top of exhaust outlet near the door ,the different parameters were adjusted and respectively set as the convention sterilization parameters and stimulated failure of critical sterilization parameters ;the qualified rates of chemical indicative cards ,the qualified rates of biological monitoring ,the cost ,and the influencing factors were observed and compared between the two kinds of packs under the two sterilization parameters .RESULTS Under the conventional sterilization parameters , both the challenge packs and the hand made test packs were qualified for discoloration of package instructions ,the qualified rates of both crawling cards and the biological culture were 100 .0% .Under the critical parameters of failure of stimulated sterilization ,the qualified rate of crawling cards of package instruction of the challenge packs was 83 .3% ,the hand made test packs 90 .0% .The qualified rate of biological culture of the challenge packs was 63 .3% ,the hand made test packs 80 .0% ,and there was no statistically significant difference in the effect of steri‐lization between the comprehensive challenge packs and the hand made test packs under the same sterilization pa‐rameter .CONCLUSION The study demonstrates that the comprehensive challenge test packs conform to the criteria in the pressure steam sterilization ,and the influencing factors for the accuracy of pressure steam sterilization of the hand made test packs are greater than those of the comprehensive challenge test packs .
