对比研究补肾祛毒汤治疗脾肾气虚型慢性肾衰的疗效

摘　　要：目的:对比研究补肾祛毒汤对脾肾气虚型慢性肾功能衰竭的疗效,明确其临床作用,探讨其作用机制。方法:选择符合纳入标准、排除标准的脾肾气虚型慢性肾衰患者68例,采用非随机同期对照方法分为两组,均在西医基础治疗前提下,治疗组予补肾祛毒汤加减口服,对照组予尿毒清颗粒治疗,时间3月。对比观察两组患者证候、血肌酐、血尿素氮、血尿酸、血清白蛋白、血红蛋白、血清转化生长因子-β1、骨形成蛋白-7等变化。结果:两组SCr、BUN、BUA、ALB、Hb、TGF-β1、BMP-7、CCr等治疗前后疗效显著(P<0.05),而治疗组对SCr、BUN、BUA、ALB、Hb、TGF-β1、BMP-7、CCr疗效极显著(P<0.01),治疗后两组SCr、BUN、ALB、CCr有显著差异(P<0.05);TGF-β1、BMP-7等级相关性差异极显著(P<0.01)。结论:补肾祛毒汤临床运用可随症加减,符合中医辨证论治的思维,其疗效较尿毒清颗粒在改善肾功能、改善肾小球滤过功能、升高血清白蛋白等方面更显著,并能减少血TGF-β1,上调血BMP-7的表达。Objective ： To research the different treatments of Bushen Qudu Decoction and Uremic Clearance Granule on the same syndrome of Qi deficiency of spleen and kidney of chronic renal failure （CRF）. And to make sure the function and mechanism of Bushen Qudu Decoction. Methods：68 patients who met the inclusion and excluded criteria were divided into treatment group and control group by a non - randomized controlled method. They were all routinely treated by the same therapeutic method while the treatment group was treated by Bushen Qudu Decoction with the other group treated by Clearance Granule. The treatment was for 3 months. We studied the floating results of the 2 groups＇ syndrome, serum creatinine （SCr） , blood urea nitrogen （BUN） , blood uric acid （BUA） , serum albumin （ALB） , hemoglobin （Hb） , transforming growth factor - β1 （ TGF - β1 ） and bone morphogenetic protein - 7 （ BMP - 7 ） . Results ： The results of the 2 groups＇ SCr, BUN, BUA, ALB, Hb, TGF - β1, BMP - 7 and CCr were all improved significantly （ P 〈 0. 05 ） and the treatment group held more significant results than the control group in SCr, BUN, BUA, ALB, Hb, TGF- β1, BMP- 7 and CCr （P 〈 0.01 ）. The post- treatment of the 2 groups showed more significant results in SCr, BUN, ALB and CCr（ P 〈 0. 05 ）. And expression of TGF - β1 showed a positive correlation with BMP - 7. Conclusions ： Bushen Qudu Decoction is used widely and can conform to the treatment according to syndrome differentiation. Its effectiveness is more significant than Uremic Clearance Granule in improving renal function, glomerular filtration function and serum albumin level. It also can reduce the expression of TGF- β1 and promote BMP- 7.
