摘 要:目的:考察中医辨证联合人工泪液治疗治疗干眼症的临床效果。方法:选取2009年1月-2010年1月我院眼科门诊的干眼症患者124例,随机分为对照组(人工泪液)和实验组(人工泪液+中医辨证),治疗1个月后,比较两组患者有效率、眼部症状、泪膜破裂时间(BUT)、泪液分泌量(SIT)、角膜荧光染色(FL)、角膜表面规则性指数(SRI)和角膜表面不对称指数(SAI)。结果:对照组总有效率57.3%,实验组总有效率79.3%,两组比较差异具有统计学意义(P<0.05);两组患者在异物感和视疲劳评分无统计学差异(P>0.05),实验组眼干涩和眼红评分显著优于对照组(-2.16±0.97vs-1.39±1.19,P<0.01;-0.97±0.60vs-0.20±0.41,P<0.01)。在FL和SIT指标中,实验组显著优于对照组(1.51±0.16vs2.02±0.18,P<0.05;4.51±0.53vs3.72±0.37,P<0.05),在BUT指标中,实验组优于对照组(5.51±0.75vs5.22±0.82,P<0.05);在SRI和SAI指标中,实验组均优于对照组(0.53±0.35vs0.66±0.41,P<0.05;0.42±0.25vs0.52±0.32,P<0.05)。结论:人工泪液联合中药可有效提高患者视功能,改善干眼病症状。Objective : To investigate the effect of artificial tears and syndrome differentiation on xerophthalmia. Methods: We selected 124 cases of xerophthalmia patients from January 2009 to January 2009 in our hospital. Those people were divided into control group (artificial tears) and experimental group (artificial tears + syndrome differentiation). After treatment for one month, we compared two groups rate of treatment, rupture time (BUT), tear secretion (SIT) and corneal fluorescence staining (FL) , corneal surface regularity index (SRI) and corneal surface asymmetry index (SAI). Results : The effective rate in control group was 57.3% and in the experimental group it was 79.3%. Between two groups there was significant difference (P 〈 0.05 ). There was no statistical difference in foreign body sensation and visual fatigue scores between two groups ( P 〉 0.05 ). As for dry eyes and envy score, experimental group was better than control group. Experimental dry eyes and envy score were significantly better than that of control group( -2.16 ±0.97vs - 1. 39 ± 1.19, P 〈 0.01 ; - 0.97 ± 0.60vs - 0.20 ±0.41, P 〈 0.01 ). As for SIT and FL, the experimental group was significantly better than the control group ( 1.51 ± 0.16 vs 2. 02 ± 0.18,P 〈 0.05 ; 4.51 ± 0.53 vs 3.72 ± 0.37, P 〈 0.05 ). As for BUT, the experimental group was better than that of the control group (5.51 ± 0.75 vs 5.22 ±0.82, P 〈 0. 05 ). As for SRI and SAI, the experimental group was better than the control group (0.53 ± 0.35 vs 0.66 ± 0.41, P 〈 0.05 ; 0. 42 ± 0.25 vs 0. 52 ± 0.32, P 〈 0.05 ). Conclusion: Artificial tears combined with traditional Chinese medicine could effectively improve patients visual function and improve xerophthalmia symptoms.
