慢性阻塞性肺疾病患者心理健康状况及其影响因素的分析研究

摘　　要：目的调查慢性阻塞性肺疾病(COPD)患者的心理状况及基本情况:患者居住地、性别、COPD级别、文化程度、BMI、手术史、高血压等,社会支持度及总体幸福感对其心理状况的影响,对COPD患者的治疗、护理提出指导性意见。方法采用整群抽样方法,对经过肺功能检测筛查为COPD的103例患者进行症状自评量表(SCL-90)、社会支持评定量表(SSRS)、总体幸福感量表(GWB)调查,并了解患者基本情况。结果COPD患者SCL-90各因子得分与全国常模差异有统计学意义(P<0.001)。性别在强迫症状、敌对、偏执、精神病性的差异具有统计学意义(P<0.05;P<0.01;P<0.001);患者来源在强迫症状、人际关系、敌对、偏执、精神病性、SCL-90总评分、阳性项数的差异有统计学意义(P<0.05;P<0.01;P<0.001);GWB评分在躯体化、强迫症状、人际关系、恐怖、焦虑、偏执、精神病性、睡眠饮食、SCL-90总评分、阳性项数的差异有统计学意义(P<0.05;P<0.01;P<0.001)。结论 COPD患者心理健康状况较差,且受性别、学历、GWB得分、病情分级等因素的影响,因此心理疏导、干预在COPD患者缓解病情,提高生活质量方面具有重要意义。【Objective】 To investigate the psychological status in patients with chronic obstructive pulmonary disease（COPD） and analyze the influence of social support status, General Well-being and basic information, in order to come up with instructive suggestions of the nursing care in patients with COPD.【Methods】 103 patients with COPD screening by lung function test were measured with SCL-90, SSRS,GWB, and we know the basic information of patients. 【Results】 The date of SCL-90 in patient with COPD was significantly higher than national norm（P〈0.001）. Sex has significantly influence on obsessive-compulsive,hostility, bigot, psychosis（P〈0.05; P〈0.01; P〈0.001）; the source of patients has significantly influence on obsessive-compulsive, interpersonal relationship, hostility, bigot, psychosis, the total grade of SCL-90, the number of positive（P〈0.05; P〈0.01; P〈0.001）; the total grade of GWB has significantly influence on somatization, obsessive-compulsive, interpersonal relationship, hostility, anxiety, bigot, psychosis, sleep and diet,the total grade of SCL-90, the number of positive（P〈0.05; P〈0.01; P〈0.001）. 【Conclusion】 It is poor of mental health status in patients with chronic obstructive pulmonary disease and effected by sex, degree ofeducation, the total grade of GWB, the rank of COPD etc. Hence, psychological counseling and intervention have great influence on the remitting of patients＇ pain and improving life quality.
