摘  要:目的探讨外阴阴道假丝酵母菌病(VVC)、复发性外阴阴道假丝酵母菌病(RVVC)、滴虫性阴道炎(TV)、细菌性阴道病(BV)阴道局部Th17细胞免疫功能状况与发病的关系。方法选取2010年10月-2012年4月就诊及进行健康体检的育龄妇女,分为VVC组(27例)、RVVC组(20例)、TV组(20例)、BV组(20)、对照组(20例)。留取阴道灌洗液,采用ELISA法检测其IL-17AF和IL-23浓度。结果 1各组间阴道灌洗液中IL-17AF表达水平:RVVC组与VVC组明显高于TV组、BV组及对照组,差异均具有显著性(P<0.01);RVVC组与VVC组之间差异无显著性(P>0.05);TV组、BV组及对照组之间的差异也没有显著性(P>0.05)。2各组间阴道灌洗液中IL-23的表达水平的差异无显著性(P>0.05)。结论阴道局部IL-17AF可能在人体抵抗假丝酵母菌的急性感染中起着重要的作用,但可能不是影响外阴阴道假丝酵母菌病反复发作的主要因素。Th17细胞在滴虫性阴道炎以及细菌性阴道病方面的作用可能是极为有限的。在女性下生殖道感染中,阴道局部IL-23可能不是Th17细胞作用的主要调节因素。【Objective】To investigate the relationship between local Th17 cellular immunity of vagina and the pathogenesis of female lower genital tract infections, including vulvovaginal candidiasis(VVC), recurrent vulvovaginal candidiasis(RVVC), trichomonal vaginitis(TV), and bacterial vaginosis(BV).【Methods】27 cases of VVC, 20 cases of RVVC, 20 cases of TV, 20 cases of BV, and 20 cases of healthy women were included in the study. The cervicovaginal lavage fluids were collected and the levels of IL-17 AF, IL-23 were measured by ELISA. 【Results】The levels of IL-17 AF in cervicovaginal lavage fluids were significantly higher in RVVC and VVC group than other groups(P〈0.01). However, no statistical difference was found between RVVC and VVC group(P〉0.05). Similarly, no statistical difference was found among TV, BV and control group(P〉0.05). No statistical difference was found between groups in terms of the levels of IL-23 in cervicovaginal lavage fluids(P〉0.05). 【Conclusions】Local IL-17 AF in the vagina may play an important role in resistance to acute infection of candida. However, it may not be a main factor in the recurrence of VVC. The role of Th17 cells may be limited in the fields of trichomonal vaginitis and bacterial vaginosis. IL-23 may not be a primary regulator to Th17 cells in the vagina locally in female lower genital tract infections.
