中风后抑郁症细胞凋亡及逍遥散加减治疗临床研究

摘　　要：目的:探讨细胞凋亡在肝郁脾虚型中风后抑郁症(PSD)发病过程中的作用和逍遥散加减治疗的疗效。方法:选择61例PSD患者,应用流式细胞仪进行检测淋巴细胞凋亡率,以逍遥散加减为主治疗,观察临床症状等级评分,与40例健康志愿者作比较。结果:肝郁脾虚型PSD组治疗前淋巴细胞凋亡率、B细胞淋巴瘤基因-2(bcl-2)表达与健康对照组比较,明显高于健康对照组(P<0.01);逍遥散加减治疗后肝郁脾虚型PSD患者外周血淋巴细胞凋亡率、B细胞淋巴瘤基因-2(bcl-2)表达明显低于治疗前,有显著性差异(P<0.05)。肝郁脾虚型PSD组治疗前后临床症状大部分有显著性差异(P<0.05,P<0.01)。结论:肝郁脾虚型PSD的发病过程存在着细胞凋亡,而且逍遥散加减治疗效果良好。Objective：To discuss the apoptosis function and syndrome differentiation and treatment in post-stroke depression（PSD） with liver depression and spleen deficiency syndrome. Method：We selected 61 cases of PSD. Flow eytometry was used in detection of lymphocyte apoptosis rates. We observed clinical symptoms scoring of PSD treated with Xiaoyao powers. There were 40 cases of healthy volunteers as the control. Results ：Lymphoid cell apoptosis rate and bcl-2 expresses of PSD group were obviously higher than that of health control group before treatment（P〈0.01）. Lymphoid cell apoptosis rate and bel-2 expresses of PSD group were obviously lower than health control group after treatment by Xiaoyao powers（P〈0.05）. There were obviously difference of clinical symptoms scoring of PSD with liver depression and spleen deficiency syndrome before and after treatment（P〈0.05,P〈0.01）. Conclusions：PSD with liver depression and spleen deficiency syndrome had cell apoptosis,and treated by Xiaoyao power had good treatment effect.
