联合检测宫颈阴道分泌物中胎儿纤维连接蛋白及白介素-6对早产的预测价值

摘　　要：目的探讨宫颈阴道分泌物中胎儿纤维连接蛋白(f FN)及白介素-6(IL-6)的联合检测对早产的预测价值。方法选取2013年2月—2014年2月在南京市妇幼保健院住院治疗的45例先兆早产孕妇设为观察组,同期正常孕妇50例设为对照组。观察2组宫颈阴道分泌物中f FN、IL-6表达水平差异,不同妊娠结局孕妇上述指标差异及其相互关系。结果观察组宫颈阴道分泌物中f FN及IL-6表达水平均显著高于对照组(P<0.01);95例孕妇中,妊娠结局为早产者29例,足月产者66例,早产孕妇上述指标显著高于足月产者(P<0.01);联合检测f FN和IL-6预测先兆早产敏感度为91.11%,特异度为42.00%;预测妊娠结局的敏感度和特异度分别为93.10%及59.09%;相关性分析表明f FN与IL-6显著正相关(r=0.839,P<0.01)。结论宫颈阴道分泌物中f FN和IL-6表达水平是预测早产的较敏感指标,联合检测可提高临床预测早产及妊娠结局的能力。Objective To explore the predictive values of combined detection of fetal fibronectin（ fFN） and interleukin-6（ IL-6） in cervical secretions for premature delivery.Methods 45 patients with threatened premature labor hospitalized in Nanjing Maternity and Child Health Care Hospital during February 2013 to February 2014 were selected as observation group and 50 normal pregnant women during the same period were selected as control group.The levels of fFN and IL-6 in the two groups were compared,and the differences of fFN and IL-6 levels between the patients of different pregnancy outcomes and their relationship were studied.Results The levels of fFN and IL-6 in observation group were statistically significantly higher than those in control group（ P〈0.01）.In 95 cases of pregnant women,29 cases were premature delivery,66 cases were mature gestation; The levels of fFN and IL-6 in premature delivery were significantly higher than those of the mature gestation（ P〈0.01）.The sensitivity of the combined detection for premature labor prediction was 91.11% and its specificity was 42.00%; its sensitivity for pregnancy outcomes was 93.10% and its specificity was 59.09%.There was a positive correlation between fFN and IL-6 level（ r = 0.839,P〈0.01）.Conclusion fFN and IL-6 levels in cervical secretions is a sensitive index for prediction of preterm delivery.Combined detection of the two indicators can improve the ability to predict the preterm labor and pregnancy outcome.
