住院失禁患者失禁性皮炎发生的相关因素分析

摘　　要：目的研究住院失禁患者发生失禁性皮炎的相关因素。方法采用横断面调查方法,对江苏省某三级综合医院的住院时间大于24 h失禁患者进行回顾性分析,记录患者的性别、年龄、意识状况、失禁频率、预防性使用皮肤保护剂情况、皮肤粉剂使用情况等6个方面可能影响失禁性皮炎发生的因素进行单因素分析,找出相关因素后再进行非条件logistic逐步回归分析。结果单因素分析共筛选出意识状况、潮湿频率、预防性使用皮肤保护剂3个方面因素差异有统计学意义(P<0.05),以是否发生失禁性皮炎为应变量,3个被筛选出因素作为自变量给予赋值后进行非条件logistic回归分析,结果显示意识状况和预防性使用皮肤保护剂2个方面因素是失禁性皮炎发生的相关因素。结论评估失禁患者失禁性皮炎发生的危险因素,关注意识障碍失禁患者皮肤护理工作,给予及时的皮肤保护剂保护皮肤,可以有效预防失禁性皮炎的发生。Objective To explore related factors of incontinence-associated dermatitis（IAD） in hospitalized patients. Methods Using cross-sectional survey method,the material of patients with incontinence more than 24 hours from level 3 general hospital of Jiangsu province were analyzed and 6 aspects including gender,age,conscious state,frequency of incontinence,use of preventive skin protectant,skin powder usage were recorded respectively. The potential influencing factors were analyzed by single factor analysis. And after finding out the related factors,unconditioned logistic regression was analyzed. Results Single factor analysis revealed that the difference in conscious state,dampness frequency and the usage of skin protectant was statistically significant（P〈0. 05）. After three factors selected as the independent variable to give unconditional logistic regression analysis after the assignment,the result showed that the two factors of awareness and preventive using skin protectant were incontinent dermatitis of related factors. Conclusion Assessment the risk factors of incontinence patients occurring incontinent dermatitis,focusing on the skin care work of patients with disturbance of consciousness incontinence and giving timely skin protection can effectively prevent the occurrence of incontinent dermatitis.
