摘  要:目的探讨矽肺患者肺泡巨噬细胞(AM)表面A类清道夫受体(SR-A)的表达及其对AM凋亡的调控作用,为矽肺的防治提供依据。方法以不同期别的矽肺患者51例为调查对象,以4例矽肺观察对象为对照组,收集其大容量肺灌洗回收液,分离、纯化AM,将纯化后的AM分为5组:未处理组、酪氨酸激酶抑制剂组、活性氧抑制剂组、抗-MARCO组和抗-MSR1组,37℃处理24h后,裂解各组AM,采用Western blot方法检测受体蛋白MSR1和MARCO及凋亡蛋白Caspase-3的表达并计算其相对水平。结果矽肺患者AM中Caspase-3的表达水平均高于对照组,且随着肺纤维化程度的加重逐渐增高(P<0.05)。MSR1和MARCO表达水平虽有随矽肺期别增高而增加的趋势,但差异无统计学意义(P>0.05)。未脱尘组Caspase-3的表达水平高于脱尘组(P<0.05);MSR1、MARCO表达水平与Caspase-3均呈正相关。抗-MARCO和抗-MSR1抗体处理后,矽肺患者Caspase-3的表达均高于未处理组,贰期矽肺患者MSR1表达高于未处理组,贰期和叁期矽肺患者MARCO的表达均较未处理组上调(P<0.05)。贰期、叁期矽肺患者的AM经N-乙酰半胱胺酸处理后Caspase-3表达水平下调。结论 SR-A在介导矽肺患者AM凋亡中可能发挥重要的作用。Objective To explore the expression of scavenger receptor of class A in the surface of alveolar macrophage(AM)from silicotic patients' lung lavage fluid and its regulating role in AM apoptosis.Methods Fifty-one silicotic patients at stage I,II and III were taken as the subjects.Four workers exposed to silica but diagnosed as suspected silicosis were taken as the controls.The lung lavage fluid of all subjects and controls were separately collected,the AMs in each sample were further separated and purified,and divided into 5groups,i.e.untreated group(A),tyrosine kinase inhibitor group(B),reactive oxygen inhibitor(N-acetylcysteine)group(C),anti-MARCO group(D)and anti-MSR1group(E).24 hours after various treatments,AMs were lysed and the levels of MSR1, MARCO and Caspase-3 were detected by Western Blot.Results The expression of Caspase-3in silicotic patients was higher than that of the controls,and increased with the severity of lung fibrosis;and the difference was significant(P〈0.05).Although MSR1 and MARCO in silicotic patients increased with the severity of silicosis,the difference showed no statistical significance(P〉0.05).Caspase-3in silica exposed group was higher than that of silica unexposed group(P〈0.05).MARCO and MSR1 showed a positive correlation to Caspase-3.The up-regulating role of anti-MSR1 and antiMARCO in Caspase-3expression appeared in all silicotic patients,MSR in silicotic patients of stage II andMARCO in patients of stage II and III(P〈0.05).The down-regulating role of N-acetylcysteine in Caspase-3showed in silicotic patients of stage II and III(P〈0.05).Conclusions Scavenger receptor class A protein MSR1,MARCO and apoptosis protein Caspase-3 may play an important role in the occurrence and development of silicosis.
