预注小剂量舒芬太尼预防舒芬太尼麻醉诱导诱发呛咳的疗效观察

摘　　要：目的观察预注小剂量舒芬太尼预防诱发呛咳的疗效及安全性,并探索最小有效剂量。方法连续选取2014年3月至1014年6月拟行全身麻醉的病例120例,随机分为4组(n=30):A组、B组、C组、D组,分别预注注射用水2ml、舒芬太尼0.1μg/kg、0.15μg/kg、0.2μg/kg。记录预注药物后及给诱导量舒芬太尼后发生呛咳的例数及严重程度,并记录生命征变化。结果在预给药后,D组呛咳程度显著高于A、B、C 3组(P<0.01)。而在给诱导量舒芬太尼后A组呛咳程度显著高于其余3组(P<0.01),其余3组则无显著性差别(P=0.889)。C组Sp O2显著低于B组(P=0.049);D组Sp O2显著低于A组(P=0.010)和B组(P=0.006)。结论提前2min预注0.1μg/kg的舒芬太尼可有效抑制快速推注0.3μg/kg舒芬太尼诱发的呛咳反应,且对呼吸的影响不大。Objective To observe the curative effect and security of pre - injecting low - dose sufentanil to prevent the patients from severe coughing and explore the minimum effective dose. Methods We chose 120 cases from March to June in 2014 where patients had received general anesthesia and divided them into four groups randomly （ n = 30） ： Group A, B, C and D, were respectively injected 2ml water for injection, 01. 1μg/kg,0. 15μg/kg and 0. 2μg/kg sufentanil. Record the cases of severe coughing and the degree after pre - injecting sufentanil and giving inducing - dose sufentanil. We recorded the changes of vital sigh at the same time. Results After pre - injecting medication, the degree of cough in Group D was distinctively higher than Group A, B and D （P 〈 0.01 ）. However, after injecting inducing - dose sufentanil, the degree of cough in Group A was distinctively higher than the other three groups （ P 〈 0. 01 ）, and there was no distinctive difference between the other three groups （P = 0. 889）. SpO2 in Group C was distinctively lower than that of Group B（P = 0. 049）. SpO2 in Group D was distinctively lower than that of Group A（ P = 0. 010） and Group B（P =0. 006）. Conclusion 0. 1μg/kg sufentanil pre- injected 2 minutes earlier can effectively prevent the cough inducing by quickly injecting 0. 3 μg/kg sufentanil, and it doesn＇ t affect breath much.
