综合康复理念在90例乳腺癌患者围术期中的应用

摘　　要：目的探讨综合康复理念在乳腺癌围术期患者中的作用。方法选取2010年11月～2011年11月于衡水市哈励逊国际和平医院收治的90例乳腺癌患者的临床资料,按护理方式分为观察组（45例）和对照组（45例）。对照组在患者围术期采用常规的护理方法,即密切监测患者病情、进行引流护理、伤口护理等。观察组在围术期常规护理的基础上,采用综合康复理念。比较两组床上活动、床下活动、住院时间等情况。监测发生呼吸系统感染、患肢淋巴水肿、切口感染、尿路感染等;并于术后1、3、6个月随访记录患者肩关节活动度、淋巴液回流及患肢功能康复情况,最后比较两组治疗效果。结果观察组患者术后一般状况明显优于对照组,观察组并发症发生率（13.33%）低于对照组（31.11%）,术后1、3、6个月肩关节活动度、淋巴液回流及患肢功能评分均高于对照组,差异均有统计学意义（P〈0.05或P〈0.01）。结论综合康复理念可以有效改善乳腺癌患者围术期的状况,同时降低并发症发生率,避免淋巴水肿发生。Objective To study the function of perioperative comprehensive rehabilitation concept on patients with breast cancer. Methods From November 2010 to November 2011, in Harrison International Peace Hospital of Hengshui City, the clinical data of 90 patients with breast cancer were selected and divided into observation group（45 cases） and control group（45 cases）, according to the nursing method. The control group was nursed with routine nursing methods,monitoring the patients closely, drainage nursing, wound nursing and so on. The observation group was nursed comprehensive rehabilitation concept based on the routine care. The bed activity, under bed activity, hospitalization time and hospitalization expenses of two groups were compared, the respiratory system infection, limb lymph edema, wound infection, urinary tract infection and so on were monitored, the situation of shoulder joint activity, lymph circumfluence and limb function recovery at 1, 3, 6 months after the surgery were recorded and compared. Results The postoperative general situation of patients in observation group was better than that of control group, complication rate of observation group（13.33%） was lower than that of control group（31.11%）, the situation grades of shoulder joint activity, lymph circumfluence and limb function recovery at 1, 3, 6 months after the surgery in observation group were better than those in control group, the differences were statistically significant（P 〈0.05 or P 〈0.01）. Conclusion Comprehensive rehabilitation concept can improve effectively situations of perioperative patients with breast cancer, reduce the complication rate of patients at the same time, and avoid the occurrence of lymphedema effectively.
