手术治疗对精索静脉曲张性重度少精或无精患者的效果

摘　　要：目的：评估精索静脉曲张高位结扎术对精索静脉曲张致重度少精或无精患者的治疗效果。方法2012年8月-2013年8月由青岛大学附属医院确诊为精索静脉曲张致重度少精（精子密度小于2百万/mL，12例）和无精患者（8例）（其中伴阴囊坠胀感5例）并行手术治疗为实验组，另选取10例精索静脉曲张致重度少精或无精且不同意手术患者作为对照组。对两组术前3d内精液分析和血清性腺激素（促卵泡刺激素、黄体生成素、睾酮、雌二醇和促泌乳素）的结果分别与术后2、4、8个月的结果进行对比分析。结果实验组患者术前与术后2、4、8个月精液分析及性腺激素比较均无明显变化，差异无统计学意义（P〉0.05），实验组与对照组比较差异亦无统计学意义（P〉0.05），但5例阴囊坠胀感患者不适症状均明显减轻或消失。结论本研究认为手术治疗对改善此类患者精液质量无明显效果，不建议对重度少精或无精并期望改善精液质量患者行手术治疗。Objective To evaluate the efficacy of high ligation of varicocele to varicocele-associated severe spermacra-sia or azoospermia patients. Methods From August 2012 to August 2013, in the Affiliated Hospital of Qingdao Univer-sity, varicocele-associated severe spermacrasia （sperm density less than 2 million/mL, 12 cases） and azoospermia pa-tients （8 cases） with high ligation of varicocele operation （5 cases with gas pains of scrotum） were selected as the ex-periment group, 10 varicocele-associated severe spermacrasiaor azoospermia patients refused high ligation of varicocele as control group. Sperm quality and gonadal hormones （FSH, LH, T, E2, PRL） of two groups were measured and the re-sults before and 2, 4, 8 months after operation were compared. Results In the experianent group, compared with 2, 4, 8 months after operation, the sperm quality and gonadal hormones before operation had no statistically significant differ-ence （P〉0.05）, but the scrotum gas pains of the 5 patients markedly release or disappeared. Compared with and con-trol group, the sperm quality and gonadal hormones of the experiment group had no significant difference （P〉 0.05）. Conclusion The high ligation of varicocele on varicocele-associated severe spermacrasia or azoospermia are inefficien-cy. It is not to recommend that these patients who want to improve their sperm quality to receive the operation.
