深静脉血栓并发症风险评估表及护理计划单在老年髋部骨折患者围手术期的应用

摘　　要：目的探讨老年髋部骨折患者围手术期下肢深静脉血栓风险评估表及护理计划单的应用效果。方法对深静脉血栓高危因素进行量化评估,内容包括年龄、基础疾病、卧床时间、疼痛、个人情况、手术时间及麻醉方式7项,根据总分值分为低度危险、中度危险和高度危险3个级别,分别在临床观察、康复功能训练、有效踝泵指导、体位及疼痛护理等方面制订并实施标准护理计划。结果深静脉血栓并发症风险评估表及护理计划单应用于老年髋部骨折患者后,268例髋部骨折患者中仅发生深静脉血栓6例。结论深静脉血栓并发症风险评估表及护理计划单的应用,可有效预防老年髋部骨折患者深静脉血栓的发生。Objective To study the effect of risk assessment and nursing plan sheet for deep vein thrombosis（DVT）complications in older patients with hip fractures in preoperative period.Methods The items of risk assessment included patients′age,primary diseases,duration of bed rest,pain,personal situation,duration of operation and anesthesia method.Based on the results of risk assessment,the patients were classified into low risk group,moderate risk group and high risk group.Then,standardized nursing plans were formulated in the aspects of clinical observation,rehabilitation training,guidance of effective ankle pump practice,position and pain management.Results After the application of the risk assessment and nursing plan sheet,only 6 cases got DVT.Conclusion The application of this sheet can effectively reduce the incidence of deep vein thrombosis complications in older patients with hip fractures.
