经尿道肾镜EMS第五代超声联合气压弹道治疗复杂性膀胱结石的临床效果观察

摘　　要：目的：探讨经尿道肾镜EMS第五代超声联合气压弹道治疗复杂性膀胱结石的临床效果。方法回顾性分析本院2010年4月～2014年6月腔镜治疗复杂性膀胱结石84例患者的临床资料,其中对照组43例采用电切镜联合输尿管镜下气压弹道碎石治疗,观察组41例采用经尿道肾镜EMS第五代超声联合气压弹道治疗,比较两组的临床效果。结果84例手术均成功,无严重出血、膀胱穿孔、导尿管脱落等并发症。对照组碎石时间为15～78 min,平均（38.7±15.2）min,观察组碎石时间为10～45 min,平均（30.5±10.2）min,观察组碎石时间短于对照组,差异有统计学意义（P=0.00）。对照组住院时间为5～16 d,平均（10.2±2.3）d,观察组为6～13 d,平均（6.2±2.4）d,差异无统计学意义（P=0.786）。结论经尿道肾镜EMS第五代超声联合气压弹道治疗复杂性膀胱结石效果显著,可明显缩短手术时间,促使患者早日康复。Objective To discuss the clinical effect of transurethral nephroscopic EMS fifth ultrasound combined with pneumatic lithotripsy in the treatment of complexity cystolith. Methods Clinical data of 84 patients with complexity cystolith treated by endoscope in our hospital from April 2010 to June 2014 were retrospectively analyzed.Resectoscope combined with ureteroscopic pneumatic lithotripsy were used in control group（43 cases）,while transurethral nephroscopic EMS fifth ultrasound combined with pneumatic lithotripsy were applied in observation group （41 cases）.The clinical effect was compared in both groups. Results Operations in eighty-four cases were all successful without complications like severe hemorrhage,bladder perforation and catheter dropping,etc.Time of lithotripsy was from 15 min to 78 min,an average lithotripsy time was （38.7±15.2）min in control group,time of lithotripsy ranged from 10 min to 45 min,average lithotripsy time was （30.5±10.2）min in observation group.The average lithotripsy time in observation group was shorter than that in control group,with a statistical difference （P=0.00）.The hospital stay ranged from 5 d to 16 d,average hospital stay was （10.2±2.3）d in control group,the hospital stay was 6 d to 13 d,average hospital stay was （6.2±2.4）d in observation group,there was no statistical difference in the hospital stay between two groups （P=0.786）. Conclusion Transurethral nephroscopic EMS fifth ultrasound combined with pneumatic lithotripsy in the treatment of complexity cystolith can obtain favorable effect,can obviously shorten operation time and promote patient’s recovery as soon as possible.
