C形臂机X线引导下侧隐窝阻滞与骶管阻滞对影像学/体征不吻合腰椎间盘突出症患者的疗效观察

摘　　要：目的:评价C形臂机X线引导下侧隐窝阻滞与骶管阻滞对影像学/体征不吻合腰椎间盘突出症患者的疗效观察。方法:影像学/体征不吻合腰椎间盘突出症患者60例,将患者随机分为两组,每组30例,C形臂机X线引导下侧隐窝阻滞组(C组)与骶管阻滞治疗组(D组)记录治疗前、治疗后3 d、1周、4周及8周VAS,治疗次数≥2例数,靶点注射/疼痛区域减轻吻合例数,穿刺成功例数,8周时疗效评定及不良反应发生情况。结果:与治疗前相比,两组患者治疗后3 d、1周、4周及8周VAS<4例数增多(P<0.05);与C组相比,D组治疗次数≥2的例数增多(P<0.05);与D组相比,C组靶点注射/疼痛区域减轻吻合例数明显增多(P<0.05);两组穿刺成功例数无明显差异;与D组相比,C组治疗后8周时疗效评定更好(P<0.05),两组均未见不良反应的发生。结论:与骶管阻滞相比,C形臂机X线引导下侧隐窝阻滞减少治疗次数,可更好的用于影像学/体征不吻合腰椎间盘突出症患者的临床治疗。Objective：To compare the efficacy of lateral recess block with that of sacral block on the patients with image/sign inconsistent lumbar intervertebral disc herniation.Methods：60 image/sign inconsistent lumbar intervertebral disc herniation were randomly divided into two groups,30 cases in each group：C-arm fluoroscopy group （group C） and sacral block group （group D）.VAS was recorded before the treatment,3 d,4 weeks and 8 weeks after treatment.Also recorded were the case of treatment number was no less than two,the number of successful target injection/block pain relieving the efficacy evaluation at 8 weeks after the treatment,and adverse reactions.Results：Compared with pretreatment,the number of VAS ＜ 4 in two group was obviously higher at the end of treatment （P ＜ 0.05）.Compared with group D,the case of treatment number no less than two was fewer after treatment.Compared with group D,the number of successful target injection/block pain relieving was significanly higher after treatment.The therapeutic efficacy was better in group C than that in group D at 8 weeks after treatment.Conclusion：The efficacy of lateral recess block under C-ann fluoroscopy was better than that of sacral block for the treatment of paitents with image/sign inconsistent lumbar Intervertebral disc herniation.
