多层螺旋CT多平面重建对急性脑外伤颅内出血的诊断价值  被引量:3

摘  要:目的评价多平面重建是否有助于提高常规横断面CT图像不易检出的急性脑外伤患者颅内的出血灶。方法选取过去12周内急性脑外伤的436急诊患者作为研究对象,所有患者行常规CT扫描,然后将图像以1.5mm进行重组,重建间隔1 mm。436例患者中有64例(14.6%)发现有外伤性颅内出血。由2名影像诊断医生对所有颅内出血的病例的出血部位及表现进行回顾性研究。结果在426例患者中有64例、208处出血灶。208处出血灶中有16例、30处(30/206、14.6%)在常规轴位图像漏诊,多平面重建后将出血灶清晰的显示出来,这些轴位容易漏诊的出血部位主要以前、中颅窝、颅顶、胼胝体、大脑镰、小脑幕和枕骨凸多见。而且有14例、28处在横断面上可疑为出血灶,经MPR图像证实不是出血。结论多平面重建图像结合常规横断面图像有助于提高对颅内出血的诊断。因此,对急性脑外伤患者头颅CT常规轴位图像分析时,结合多平面重建图像进行分析是有必要的。Objective:The purpose of this study was to evaluate whether multiplanar reconstruction( MPR)help to im-prove the conventional axial CT images easily detected in patients with acute brain injury with intracranial hemorrhagic le-sions. Methods:Select the last 4 weeks of emergency patients with acute brain injury as the object of study,all patients with CT scan,and then re - image the composition 1. 5mm,reconstruction interval:0. 75mm. 64 cases in 436 patients ( 14. 6%)found traumatic intracranial hemorrhage. Imaging by the two doctors of all cases of intracranial hemorrhage loca-tion and the performance of bleeding were studied retrospectively. Results:In 436 cases of patients,including 64 cases of intracranial hemorrhage(a total of 208 hemorrhagic lesions). 208 hemorrhage in 30 only through the MPR(14. 6%)(16 cases)detected. They are mainly distributed in the former,the middle cranial fossa,parietal,corpus callosum,falx,ten-torium and occipital convexity. And 28 instances(14cases)in the cross section of suspicious lesions to hemorrhage,con-firmed by the MPR image is not a hemorrhage. Conclusion:MPR images combined with conventional cross-section will help improve the diagnosis of ICH. Therefore,it is necessary to acute MPR reconstruction of traumatic brain injury patients.
