脑梗死患者血清尿酸水平与颈动脉粥样硬化斑块危险因素的关系

摘　　要：目的 探讨脑梗死患者血清尿酸水平与颈动脉粥样硬化斑块危险因素的关系.方法 选择脑梗死患者181例,根据颈动脉超声检查分为无斑块组59例,稳定斑块组48例,易损斑块组74例.检测血清尿酸水平,双功能彩色多普勒超声检测颈动脉内膜中层厚度及斑块性质.结果 易损斑块组血清尿酸水平明显高于无斑块组和稳定斑块组[（396.26±126.84）μmol/L vs （353.75±126.25） μmol/L,（325.77±94.08）μmol/L,P＜0.05].血清尿酸与颈动脉内膜中层厚度和TG呈正相关,与HDL-C呈负相关（P＜0.01）.血清尿酸是脑梗死患者颈动脉易损斑块的独立危险因素（OR=1.006,95％CI：1.002-1.101,P=0.002）.结论 血清尿酸是脑梗死患者动脉粥样硬化和斑块形成的危险因素,是易损斑块形成的可能独立危险因素.Objective To study the relationship between serum uric acid （UA） level and risk factors for carotid atherosclerotic plaques in ischemic stroke patients.Methods One hundred and eighty-one ischemic stroke patients were divided into non-plaque group （n =59）,stable plaque group （n=48） and vulnerable plaque group （n=74） according to their carotid ultrasonographic findings.Their serum UA level was measured,and their carotid IMT and plaque features were detected by color Doppler ultrasonograpgy.Results The serum UA level was significantly higher in vulnerable plaque group than in non-plaque group and stable plaque group （396.26±126.84 μ mol/L vs 353.75± 126.25 μmol/L,325.77±94.08 μ mol/L,P＜0.05）.The serum UA level was positively related with carotid IMT and TG and negatively related with HDL-C （P＜0.01）.Logistic regression analysis revealed that the serum UA level was an independent risk factor for carotid vulnerable plaques （OR=1.006,95％CI：1.002-1.101,P=0.002）.Conclusion Serum UA level is a risk factor for carotid atherosclerotic plaques and an independent risk factor for vulnerable plaques in ischemic stroke patients.
