摘  要:目的 探讨行冠状动脉造影检查的冠心病患者血清胱抑素C(CysC)水平是否能预测冠状动脉病变严重程度.方法 选择行冠状动脉造影的胸痛患者435例.67例患者证实冠状动脉狭窄<50%为对照组;余368例为冠心病组,又根据SYNTAX评分结果分为低分组(0-22分)134例、中分组(23-32分)177例和高分组(>32分)57例.造影前均检测血清CysC.结果 与低分组比较,中分组和高分组CysC明显升高(11.49士4.81) μmol/L和(13.63土5.30)μmmol/L vs (14.35±3.37)μmmol/L,P<0.05).CysC与SYNTAX评分呈正相关(r=0.106,P=0.001).单因素logistic回归分析显示,年龄、糖尿病和血清肌酐是SYNTAX评分>32分最强预测因素(P<0.01).多因素logistic回归分析显示,年龄和糖尿病是SYNTAX评分>32分最强预测因素(P<0.01),血清CysC及空腹血糖是其独立预测因素(P<0.05).结论 血清CysC与冠状动脉病变严重程度密切相关,随着血清CysC水平升高,冠心病病情逐渐加重.Objective To study whether serum cystatin C (CysC) level can predict the severity of coronary artery lesion in coronary heart disease (CHD) patients undergoing coronary arteriography.Methods Four hundred and thirty-five chest pain patients who underwent coronary arteriography (CAG) were divided into control group with their coronary stenosis <50% (n=67) and CHD group(n=368),and into 0-22 SYNTAX score group (n=134),23-32 SYNTAX score group (n=177) and >32 SYNTAX score group (n=57).Their serum CysC level was measured before CAG.Results The serum CysC level was significantly higher in 23-32 SYNTAX score group and >32 SYNTAX score group than in 0-22 SYNTAX score group (14.35±3.37μmol/L vs 11.49±4.81 μmol/L,13.63±5.30 μmmol/L vs 14.35±3.37 μmol/L,P<0.05).Univariate logistic regression analysis showed that age and diabetes mellitus were the most effective predictors whereas serum CysC and fasting blood glucose levels were the independent predictors for coronary artery lesion with a SYNTAX score >32 (P<0.05,P<0.01).Conclusion Serum CysC level is closely associated with the severity of coronary artery lesion and CHD exacerbates with the elevated CysC level.
