摘 要:目的观察瑞舒伐他汀对慢性心力衰竭(CHF)大鼠中心肌细胞骨保护素/核因子κB受体活化因子配体(OPG/RANKL)表达的影响。方法 70只雄性SD大鼠中,随机选择58只采用腹主动脉缩窄法制备CHF模型,将存活的48只CHF大鼠随机分为未治疗组、低剂量瑞舒伐他汀组(低剂量组)和高剂量瑞舒伐他汀组(高剂量组),各16只,另12只作为假手术组。于造模4周及给药4周后行超声心动图检查后处死,观察心肌组织病理学变化,测定心肌组织OPG、RANKL mRNA表达。结果给药4周后,与未治疗组和低剂量组比较,高剂量组大鼠左心室收缩末内径明显降低,LVEF、左心室短轴缩短率明显升高(P<0.05)。光镜下,高剂量组心肌损伤程度较未治疗组及低剂量组减轻。与假手术组比较,未治疗组、低剂量组、高剂量组大鼠心肌组织OPG、RANKL mRNA表达明显增加(P<0.01),RANKL/OPG值增高(P<0.01);与未治疗组及低剂量组比较,高剂量组心肌组织RANKL mRNA表达下调(P<0.05),RANKL/OPG值降低(P<0.05)。结论 CHF大鼠心肌细胞OPG、RANKL表达增强。高剂量瑞舒伐他汀可能通过下调RANKL表达抑制心室重构,改善心功能,延缓CHF进展。Objective To study the effect of rosuvastatin on expression of osteoprotegerin/nuclear factor κB receptor activator ligand (OPG/RANKL) in myocardial cells of CHF rats.Methods A rat CHF model of was established by abdominal aorta occlusion.The remaining 48 rats were randomly divided into non-treatment group (group 1),low dose rosuvastatin treatment group (group 2) and high dose rosuvastatin treatment group (group 3,16 in each group).Another 12 rats served as a sham operation group (group 4).The animals underwent echocardiography and killed.Pathological changes in their myocardial tissue were observed.Expression of OPG and RANKL mRNA in myocardial tissue was detected by RT-PCR.Results The LVESD was significantly shorter while the LVEF and LVFS were significantly higher in group 3 than in group 1 and group 2 after 4 weeks of drug tratment (P<0.01).Optical microscopy showed that the myocardial damage was milder in group 3 than in group 1 and group 2.The expression levels of OPG and RANKL mRNA and the RANKL/OPG value were significantly higher in group 1,group 2 and group 3 than in group 4 (P<0.01) and significantly lower in group 3 than in group 1 and group 2 (P<0.05).Conclusion The expression of OPG and RANKL is upregulated in myocardial cells of CHF rats.High dose rosuvastatin can suppress cardiac remodeling,improve cardiac function and delay CHF progress by downregulating the expression of OPG and RANKL.
