运动康复对中重度慢性阻塞性肺疾病患者生活质量的影响

摘　　要：【目的】观察护理干预方案对中重度慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者呼吸困难焦虑症状的影响。【方法】选择2012年1至12月期间,在我院门诊和病房就诊的所有诊断为COPD的115例患者,随机分为护理干预数量不同的3组(对照组、训练组、强压训练组)。使用改良的borg量表分别测定第2、第6和第12个月增量平板运动试验的焦虑症状。【结果】强压训练组(2.18±0.19)相比较只有4次护理干预的训练组(3.06±0.75)和只进行家庭步行治疗的对照组(3.18±0.25),呼吸困难焦虑症状明显减轻,但这种下降趋势仅仅持续了6个月。【结论】呼吸困难的患者焦虑症状的改善和联合护理干预的自我管理计划有密切联系,应该得到进一步的研究。【Objective】To observe the influence of nursing intervention on dyspnea-related anxiety（DA） in patient with stable moderate and severe chronic obstructive pulmonary disease（COPD）.【Methods】115 patients in our hospital from January 2012 to December2012 were randomly selected and divided into three groups（control group,training group,and intensive training group）.The control group was given regular nursing care,while the research group was given comprehensive nursing interventions based on regular nursing care.The measurements of the DA was rated during treadmill exercise tests on two baseline days and at the second,the sixth,and the twelfth month.【Results】After the nursing,the intensive training group who received 24 nurse-coached exercise sessions was significantly better than with those who had four exercise sessions（training group） or only received a home-walking program（control group）,the intensive training group（2.18±0.19）vs training group（3.06±0.75）and control group（3.18±0.25）.Reductions in DA in the intensive training group were only maintained through six months.【Conclusion】These findings provide initial support for the positive impact of a self-management program with nurse-coached exercise on the dyspnea-related anxiety.Further investigation of interventions should be encouraged.
