手法复位配合推拿整脊治疗颈源性眩晕疗效观察

摘　　要：【目的】观察手法复位配合推拿整脊治疗颈源性眩晕的临床疗效。【方法】将84例颈源性眩晕患者随机分为治疗组和对照组各42例,治疗组采用手法复位配合推拿整脊,手法复位1次/2 d,推拿整脊1次/d;对照组采用5%葡萄糖注射液500 ml+银杏叶提取物注射液70 mg,1次/d。两组均治疗14 d为1个疗程。分别在治疗前、治疗1个疗程后经颅多普勒(transcranial doppler,TCD)超声检查。观察椎-基底动脉系统(vertebasilar artrties,VBA)的收缩峰速度(velocity peak,PK)值,椎动脉(vertebral artery,BA),基底动脉(basilar arery,BA)及小脑后下动脉(posterior Inferior cerebellar areries,PINCA)治疗前、后的结果,进行对比观察两组的临床疗效。【结果】两组临床效果均有统计学意义(P<0.05),但治疗组疗效明显优于对照组(P<0.05);两组的总有效率分别为97.61%(治疗组)和80.95%(对照组),疗效差异具有统计学意义(P<0.05)。【结论】整脊复位法治疗颈源性眩晕临床疗效优于西药方法治疗。【Objective】To observe the clinical effect of the treatment of cervical vertigo by manual reduction combined with chiropractic tendon.【Methods】84 cases of patients with cervical vertigo were randomly divided into the treatment group and the control group,with42 cases in each group,the treatment group were treated with manipulative reduction combined with massage tendon,gimmick reset 1time every two days,massage and tendon 1 times a day,the control group using 5% glucose injection 500 ml and extract of Ginkgo biloba injection 70 mg,1 time a day.The two groups were treated for two weeks as a course.Respectively,before treatment and after a course of treatment were checked with transcranial Doppler（TCD） ultrasonography.To observe the peak systolic velocity（Velocity peak,PK） of vertebro basilar artery system（VBA）,vertebral artery（Vertebral Artery,BA） and basilar artery（Basilar Arery,BA） and the posterior inferior cerebellar artery（Posterior Inferior cerebellar areries,PINCA） before and after treatment were also observed and compared the clinical efficacy results.【Results】There are significant clinical effects in the two groups（P〈0.05）,but the curative effect of treatment group was better than that of the control group（comparison between groups P〈0.05）; in the two groups,the total effective rate was 97.61%（treatment group） and 80.95%（control group）,statistically,there are differences between the two groups（P〈0.05）.【Conclusion】Chiropractic manipulation in treating cervical vertigo clinical curative effect is better than that of Western medicine.
