摘 要:目的 观察电刺激迷走神经对内毒素血症大鼠内皮多糖蛋白包被(EG)的影响.方法 成年雄性SD大鼠30只,体质量250 - 300 g,随机分为3组:生理盐水组(S组)、内毒素血症组(LPS组)和迷走神经电刺激组(VNS组),每组10只.各组大鼠建模成功4h后均做以下检查:动脉血肿瘤坏死因子-α(TNF-α)浓度;动脉血EG脱落标志物蛋白聚糖-1(Syn-1)、透明质酸(HA)和硫酸化素(HS)浓度;电镜观察小肠肠系膜小静脉EG的变化.结果 S组血浆TNF-α、Syn-1、HA和HS浓度分别为:(34.14 ±4.94) ng/L、(53.8±17.09)、(52.22±15.15)、(0.61 ±0.14) μg/L;LPS组上述4种物质浓度分别为:(222.82±13.44) ng/L、(95.10±33.54)、(108.43 ±23.66)、(2.40±1.15) μg/L,较S组显著升高(P< 0.01);VNS组上述4种物质浓度分别为:(103.04±7.18) ng/L、(59.32±18.65)、(58.8 ±15.67)、(1.00±0.15) μg/L,显著低于LPS组(P<0.01).电镜下可观察到S组大鼠肠系膜小静脉具有完整连续的EG,而LPS组EG脱落明显,VNS组则未见明显EG脱落.结论 电刺激迷走神经对内毒素血症大鼠的内皮多糖蛋白包被可产生保护作用,其机制可能与胆碱能抗炎通路拮抗全身性炎性反应有关.Objective To observe the effects of electrical vagus nerve stimulation on endothelial glycocalyx in rat with endotoxemia.Methods Thirty male Sprague-Dawley rats (weigh,250-300 g) were randomly divided into 3 groups:normal saline group (group S),endotoxemia group (group LPS) and vagus nerve stimulation group (group VNS),each group had 10 rats.After the models of each group were built 4 h later,all samples of blood and electron microscope were collected.The following measurements were performed in all groups:the concentration of arterial plasma tumor necrosis factor-α (TNF-α) ; the concentrations of arterial plasma hyaluronan (HA),syndecan-1 (Syn-1) and heparin sulfate (HS),which are valid markers for the shedding of glycocalyx; the venules of the small bowel mesentery were identified using electron microscope to observe the changes of endothelial glycocalyx.Results In group S,the concentration of TNF-α,HA,Syn-1 and HS were (34.14 ± 4.94) ng/L,(53.8 ± 17.09),(52.22 ± 15.15),(0.61 ±0.14) μag/L; In group LPS,the concentration of them were (222.82 ± 13.44) ng/L,(95.10±33.54),(108.43 ±23.66),(2.40 ± 1.15) μg/L,which had a significantly increase in group LPS (P 〈0.01) ; while compared with group LPS,the concentration of them in group VNS were (103.04 ± 7.18) ng/L,(59.32 ± 18.65),(58.8 ± 15.67),(1.00 ±0.15) lμg/L,which were significantly lower (P 〈0.01),Electron microscopic images revealed that group S had an intact and continuous glycocalyx which was virtually ablated in group LPS.Shedding of glycocalyx in group VNS was not obvious.Conclusion Our results showed that electrical vagus nerve stimulation could protect endothelial glycocalyx in endotoxemia rats.Activation of cholinergic anti-inflammatory pathway,and inhibition of inflammatory responses may be involved in the mechanism.
