摘 要:目的观察外排泵抑制剂改变鲍曼不动杆菌耐药的情况,提高泛耐药鲍曼不动杆菌的治疗效果。方法通过微量肉汤稀释法测定四环素、诺氟沙星、庆大霉素、头孢噻肟抗菌药物的最低抑菌浓度,比较加入羰基氰氯苯腙(CCCP)泵抑制剂前后96株鲍曼不动杆菌对抗菌药物MIC的变化,以加入泵抑制剂后MIC值比不加泵抑制剂降低4倍或以上作为外排泵表型阳性的判定标准。结果 96株多重耐药及泛耐药鲍曼不动杆菌在加入CCCP前后,庆大霉素耐药率降低65.63%,耐药菌株数减少63株,其余四环素、诺氟沙星、头孢噻肟MIC值降低4倍及以上的分别有32株、41株、29株。最终进一步筛选出34株对4种抗生素MIC值均降低4倍及以上的菌株(35.42%)。结论泵抑制剂CCCP对于逆转鲍曼不动杆菌对四环素、诺氟沙星、庆大霉素、头孢噻肟等4种抗菌药物的耐药性起到重要作用。Objective To observe the efflux pump inhibitor change of Acinetobacter Baumannii resistance, and improve the effect of generic drug resistant Acinetobacter Baumannii treatment. Methods To determinate minimal inhibitory concentrations (MIC) of antimicrobial agents tetracycline, norfloxacin, gentamycin, cefotaxime by trace broth dilution method, by comparing to join carbonyl cyanide chlorobenzene hydrazone(CCCP) before and after pump inhibitors, the change of 96 strains acinetobact- er baumannii's MIC. antibacterial agents to join afterpump inhibitors MIC value than without pump inhibitors reduce 4 times or more as an effiux pump phenotypic positive decision criteria. Results Before and after adding 96 strains multi drug resistance and pan drug resistant Acinetobacter Banmanii to CCCP, gentamicin resistance rate decreased by 65.63%, the number of re- sistant bacteria rreduced 63 strains, the rest of tetracycline, norfloxacin, cefotaxime and MIC value decreased 4 times and above were 32 strains, 41 strains, 29 strains respectively. Finally further screened 34 strains (35.42%) whose MIC for the four kinds of antibiotics all decreased by 4 times. Conclusion Pump inhibitor CCCPplays an important rele for the reversal of Acinetobacter Baumaniig drug resistance to tetracycline, norfloxacin. gentamicin. cefotaxime.
