摘 要:目的:研究骨髓间充质干细胞(bone marrow mesenchymal stem cells,BMSCs)对糖尿病肾病大鼠足细胞相关分子podocin表达的影响及其对肾脏的保护作用。方法:将清洁级雄性SD大鼠52只随机分为正常组(n=13)、造模组(n=39),正常组给予普通饲料,造模组则高脂高糖喂养,8周后造模组一次性腹腔注射链脲佐菌素(streptozocin,STZ)35 mg·kg-1,糖尿病肾病造模成功后将造模组随机分为BMSCs组(n=13)、前列地尔组(n=13)及DN组(n=13)。于造模成功的当天,BMSCs组大鼠经尾静脉注射经体外培养、鉴定、Brdu标记后的BMSCs 2×106·ml-1,前列地尔组大鼠经腹腔注射前列地尔4μg·kg-1·d-1,而DN组及正常组则注射同等剂量的生理盐水。于注入后的1、14、28 d检测4组大鼠24 h尿蛋白、血尿素氮(BUN)和肌酐(Scr)水平;光镜下观察肾脏组织病理变化,荧光显微镜下观察BMSCs在肾脏的定位;利用免疫组化、Western-blotting检测肾组织podocin的表达。结果:BMSCs移植后28 d,与DN组和前列地尔组相比,BMSCs组大鼠尿蛋白、Scr、BUN水平均有下降(P<0.05);BMSCs组podocin的表达较正常组降低,但较DN组和前列地尔组明显上升(P<0.05);BMSCs组大鼠肾组织可见Brdu标记的阳性细胞。结论:BMSCs可以上调足细胞相关分子podocin的表达,改善足细胞融合,并促进足细胞再生,从而减少尿蛋白,对肾脏起到一种保护作用。ed molecules Dawley (SD) Objective: To investigate the effects of BMSCs transplantation on the expression of podocyte-associat- podocin in rats with diabetic nephropathy and the protection of the kidney. Methods: 52 Sprague male rats were randomly assigned into two groups: the normal group( n = 13 ) and the model group (n = 39 ) , the normal group were fed with ordinary feedstuff, the model group were fed with high sucrose, high fat and high energy feed for 8 weeks and then they would be injected with streptozocin(35 mg · kg^-1). After the dia- betic nephropathy models were affirmed,the model group were randomly assigned into three groups: BMSCs group ( n = 13 ), diabetic nephropathy group ( DN group, n = 13 ), alprostadil group ( n = 13 ). BMSCs were cultured , iden- tified and labeled by Brdu in vitro,which were then transplanted to the BMSCs group rats via the tail vein injection (2 × 106 · ml-1 ). The alprostadil group was given alprostadil injection(4 μg · kg^-1 · d^-1 ). And the diabetic ne- phropathy group and the normal group were treated with an equal volume of saline. The 24 h urinary protein, serum creatinine(Scr), and blood urea nitrogen (BUN) were tested at l d, 14 d, 28 d after the transplantation. The pathological changes in renal tissues were observed under light microscope and distribution of cells in the kidney were examined by fluorescence microscopy. The expression of podocin were detected by immunohistochemistry and Western-blotting. Results: The level of 24 h urinary protein, BUN and Scr in BMSCs group declined significantly at 28 d after transplantation, compared with diabetic nephropathy group and alprostadil group (P 〈 0. 05 ). The ex- pression of podocin in BMSCs group were decreased compared with normal group, but increased compared with DN group and alprostadil group(P 〈0.05).
