瘿瘤方治疗气滞痰凝型结节性甲状腺肿的临床随机对照研究

摘　　要：目的观察瘿瘤方治疗气滞痰凝型结节性甲状腺肿的临床疗效及其对甲状腺功能的影响。方法将60例气滞痰凝型结节性甲状腺肿患者随机分为治疗组和对照组,每组30例。对照组口服左甲状腺素,治疗组予瘿瘤方。两组疗程均为3个月,观察临床疗效,以及中医证候积分、血清甲状腺素水平、甲状腺结节大小的变化情况。结果 1治疗组、对照组总有效率分别为83.3%、70.0%,组间临床疗效差异无统计学意义(P>0.05)。2治疗前后组内比较,两组胸闷腹胀、身重不爽、腹部肥满、上眼睑肿、口腔黏腻、痰多积分均明显减少(P<0.05),治疗组在减少口腔黏腻积分方面优于对照组(P<0.05)。3治疗前后组内比较,两组TSH水平均明显下降(P<0.05);组间治疗后比较,甲状腺激素水平差异无统计学意义(P>0.05)。4治疗前后组内比较,治疗组甲状腺结节最大径明显缩小(P<0.05),对照组无明显变化(P>0.05);组间治疗后比较,治疗组甲状腺结节最大径小于对照组(P<0.05)。结论瘿瘤方可显著降低结节性甲状腺肿患者血清TSH浓度,缩小结节,改善临床证候。Objectlve To observe the clinical efficacy of ＂Yingliu Formula＂ for the treatment of nodular goiter and to explore its effects on thyroid function. Methods Sixty cases were rafidomly divided into two groups, with 30 patients in each group. Control group was treated by Levothyroxinc orally, and the treatment group was treated by ＂Yingliu Formula＂, with the course of 3 months. The clinical efficacy, TCM symptom score, level of serum thyroxine and size of thyroid nodules were observed. Results （1） The total effective rate was 83.3% in the treatment group and 70.0% in the control group, without significant difference between the two groups （P 〉0.05）. （2） The chest distress, heavy body, fat abdomen, swollen upper eyelid, greasy oral cavity and excessive phlegm score were reduced significantly in both groups （ P 〈0.05 ）. Treatment group was superior to the control group in relieving greasy oral cavity （ P 〈0.05 ）. （3） TSH level was markedly reduced in both groups between before and after treatment （P 〈 0.05 ） ; after treatment, no significant difference was found in thyroid hormone level between the two groups （ P 〉 0.05 ）. （4） The size of thyroid nodules were reduced in the treatment group （ P 〈 0.05 ）, without signficnat difference in the control group （P 〉 0. 05 ） ; after treatment, the size of thyroid nodules in the treatment group were less than that of the controls （ P 〈 0.05 ）. Conclusion ＂Yingliu Formula＂ can reduce TSH level, shrink the thyroid nodules to improve the clinical symptoms of nodular goiter.
