电针配合点穴推拿治疗腰椎间盘突出症疼痛的临床随机对照研究

摘　　要：目的评价电针配合点穴推拿治疗腰椎间盘突出症疼痛的临床疗效。方法将90例腰椎间盘突出症疼痛患者随机分为电针配合点穴推拿组、单纯电针组、单纯点穴推拿组,每组30例。单纯电针组选择气海俞、大肠俞、关元俞、环跳、委中、承山等穴位进行电针治疗,单纯点穴推拿组选择足太阳膀胱经和华佗夹脊穴点穴推拿,电针配合点穴推拿组予电针刺激气海俞、大肠俞、关元俞、环跳、委中、承山以及点穴推拿足太阳膀胱经和华佗夹脊穴。各组均每两天治疗1次,疗程为5次,采用Mc Gill疼痛问卷观察患者疼痛情况。结果治疗后,各组PRI、VAS、PPI积分显著减少(P<0.001);治疗前后差值比较,电针配合点穴推拿组与其余两组PRI、VAS、PPI积分差异有统计学意义(P<0.05),单纯电针组、单纯点穴推拿组之间各指标差异均无统计学意义(P>0.05)。结论单纯电针、单纯点穴推拿、电针配合点穴推拿三种治疗方法均能缓解腰椎间盘突出症疼痛症状,但电针配合点穴推拿疗效更好。Objective To observe the clinical efficacy of electroacupuncture and acupoint massage for the treatment of pain of lumbar disc herniation. Methods Ninety cases with lumbar disc herniation were randomized into three groups ： integrated group, electroacupuncture group and acupoint massage group, with 30 cases in each group. Electroacupuncture group was treated by needling on Qihaishu （ BL24 ） , Dachanshu （ BL25 ）, Guanyuanshu （ BL26 ） , Huantiao （ GB 30 ） , Weizhong （ BL 40 ）, Chengshan （ BL57 ） and so on. Acupoint massage group was treated by aeupoint pressing therapy on bladder meridian and Hua Tuo＇ s paravertebral points. Once in every two days, with the course of 5 times. The McGin Pain Questionnaire was applied. Results After treatment, the scores of PRI, VAS and PPI in each group was reduced significantly （ P 〈0. 001 ） ; we compared the difference value before and after treatment, the scores of PRI, VAS and PPI of the integrated group has significant difference from that of the electroacupuncture group and aeupoint massage group （ P 〈0.05 ）, no significant difference was found between the electroacupuncture group and acupoint massage group （ P 〉0.05 ）. Conclusion All these three different methods have positive effect on pain of lumbar disc herniation, and combined electroaeupuncture and acupoint massage has better therapeutic effect.
