摘　　要：目的：了解我院腹腔镜胆囊切除术围手术期预防性抗菌药使用情况,为规范该类手术抗菌药使用提供依据。方法：采用回顾性调查方法,对2013年6-12月232例行腹腔镜胆囊切除术的出院病例抗菌药使用情况、术后感染率、费用等指标进行统计分析。结果：抗菌药预防性应用率为15%,平均用药时间为2.1 d,最长用药时间为4 d;预防性应用抗菌药共涉及4类6种,应用频次排序前3位的依次为头孢西酮、头孢曲松/他唑巴坦、头孢唑林。术后有1例患者发生感染。平均预防性应用抗菌药费用占平均总药费的30.1%。结论：我院腹腔镜胆囊切除术预防性抗菌药使用率为15%,低于原卫生部关于Ⅰ类切口抗菌药使用率小于30%的要求,但在用药病例中存在药物选择、用药疗程、给药时机等方面不合理现象,需进一步加强监督和管理。Objective： To provide the regulate in use of antimicrobial drugs for such operations by analyzing the perioperative use of antimicrobials in laparoscopic cholecystectomy in our hospital. Methods：Retrospective study of the pa tients undergoing laparoscopic cholecystectomy section 232 cases from June to December in 2013, investigation and analysis of the use of antibiotics, postoperative infection, fees and so on. Results：Prophylactic antibiotics was 15%, with an average treatment time of 2.1 days, the longest treatment time was 4d; prophylactic antimierobials involves four categories of six kinds of application frecluency sort of front three were cefazedone, ceftriaxone tazobactam, cefazolin. One patient occurred postoperative infection. The average cost of prophylactic antimicrobial drugs accounted for 30.1% of the average total drug costs. Conclusion： Our hospital laparoscopic cholecystectomy preventive antimicrobial agents usage was 15%, lower than the ministry of health on incision I type antimicrobial usage and is less than 30% of the requirements. But the drug of choice, drug treatment, the timing of administration were not reasonable. The supervision and management need to be fur ther strengthen.
