摘 要:目的分析低浓度盐酸罗哌卡因行腰-硬联合麻醉在膝关节镜手术中的应用价值。方法随机将60例拟行关节镜手术的患者随机分为A、B两组,每组30例患者,两组均采用腰-硬联合麻醉。A组麻醉药物给予浓度为0.5%的盐酸罗哌卡因,B组麻醉药物给予浓度为0.25%的盐酸罗哌卡因,对两组的麻醉效果及麻醉安全性进行分析。结果两组感觉阻滞起效时间、感觉阻滞维持时间、平面固定时间、运动阻滞起效时间、运动阻滞维持时间及达到BromageⅠ级、Ⅱ级及Ⅲ级的时间比较,差异无统计学意义(P>0.05);B组术后感觉及运动阻滞恢复时间均明显优于A组(P<0.05)。两组麻醉前HR、SBP及DBP比较未见统计学意义(P>0.05),自患者接受麻醉5 min开始至手术结束时A组患者HR、SBP及DBP均较麻醉前明显下降(P<0.05),而B组患者则较麻醉前无明显改变(P>0.05);B组患者麻醉中不良反应发生率明显低于A组(P<0.05)。结论膝关节镜手术中应用低浓度盐酸罗哌卡因行腰-硬联合麻醉有较好的麻醉效果,且可降低麻醉不良反应发生率。Objective To analyze the value of low concentration of hydrochloric acid pp line waist-hard combined anesthesia in the arthroscopic surgery. Methods Sixty cases of arthroscopic surgery patients were random Ly divided into A、B group, 30 patients in each group. All patients were given waist-hard combined anesthesia. Group A was given concentration of hydrochloric acid with 0.5% hydrochloric acid, pp paid, group B was given concentration of hydrochloric acid with 0.25% hydrochloric acid, pp paid. Analyzed the effect of two groups. Results The sensory blockade effect time, sensory block to maintain time, plane fixed time, sports anesthesia work time, motion block maintain time and achieve Bromage Ⅰ, Ⅱ and Ⅲ class time of two groups had no statistical difference(P〉0.05); The recovery time of sensory and motor block in B group were better than group A P〈0.05). There was no difference of HR, SBP and DBP between two groups before anesthesia(P〉0.05), The HR, SBP and DBP of group A were relatively decreased than group B since the end of 5 min to anesthesia surgery(P〈0.05), while group B patients was no obvious change before anesthesia(P〉0.05). The incidence of adverse reactions in group B was obviously lower than group A(P〈0.05). Conclusion There also has good anesthesia effect for the arthroscopic surgery use low concentration hydrochloric acid in whit waisthard combined anesthesia, and can decrease the incidence of adverse reactions to anesthesia effect.
