独活寄生汤联合外用熏洗治疗寒湿腰痛60例临床观察

摘　　要：[目的]观察独活寄生汤联合外用熏洗治疗寒湿腰痛疗效。[方法]使用前瞻性设计方法,对60例门诊患者使用独活寄生汤（茯苓、独活、当归、秦艽、杜仲、川芎各10g,肉桂3-5g,人参、牛膝各10g,细辛3-6g,桑寄生15-20g,生地12-15g,白芍10-15g,甘草6g,干姜8g）,痛感剧烈加制草乌、川乌、白花蛇,活血止痛、搜风通络;湿邪偏盛去生地,加苡仁、防己、苍术、九节茶,祛湿消肿止痛;寒邪偏盛加附子,温阳散寒;脊柱疼痛明显加地鳖虫、金毛狗脊、羌活;腰膝关节骨刺明显去杜仲,加补骨脂、骨碎补、没药、乳香、生龙骨。水煎300mL,1剂/d,早晚分服。外用熏洗方（红花、当归、制草乌、制川乌、川椒、露蜂房、麻黄、桂枝）,煎煮取汁,30min/次,2次/d。连续治疗7d为1疗程。观测临床症状、不良反应。治疗1疗程,判定疗效。[结果]痊愈56例,显效2例,有效1例,总有效率98.33%。[结论]独活寄生汤联合外用熏洗治疗寒湿腰痛效果显著,值得推广。[ Objective ] To observe the living by parasitic soup combined external use fumigation curative effect and dampness lumbago. [ Methods ] Using the prospective design method, 60 cases of clinic patients living by using parasitic soup （ poria cocos, live alone, angelica, radix gentianae macrophyllae, eucommia bark, rhizoma lignstici wallichii each 10 g, cinnamon, 3 - 5 g, ginseng, twotooth achyranthes root, 10 g each, asarum 3 - 6 g, mulberry parasitism 15 - 20 g, 12 - 15 g radix rehmanniae, radix paeoniae alba 10 - 15 g, licorice root 6 g, dried ginger, 8 g ）, severe pain and kusnezoff monkshood root, radix aconiti, white snake, invigorate the circulation of the pain, for the wind t2dm;Sheng to radix rehmanniae origin, plus ＇s-tears, prevention, rhizoma atractylodis, nine section tea, qushi acetanilide detumescence; Cold evil remedying of aconite, this cold ; Spinal pain significantly add ground beetle insect, golden retriever dog ridge, notopterygium;Waist and knee joint bone spur significantly to eucommia bark, malaytea scurfpea fruit, rhizoma drynariae, myrrh, frankincense, keel. Boiling water 300 ml, 1 / d, points in the morning and evening. External use fumigation party （ safflower, Chinese angelica, kusnezoff monkshood root, radix aconiti, sichuan pepper, hives, ephedra, cassia twig ）, decoction juice, 30 rain/times, 2 times/d. Continuous treatment of 7 d for 1 course of treatment. Observation of clinical symptoms and adverse reactions. As a course treatment, judge curative effect. [ Results ] 56 cases cured, 2 cases had marked effect, 1 case effectively, the total effective rate was 98.33%. [ Conclusion ] Live alone parasitic soup combined external use fumigation treatment of cold dampness lumbago effect is remarkable, is worth promoting.
