辨证分型联合西药治疗缺血型视网膜中央静脉栓塞随机平行对照研究  

Randomized and Paralleled Control Study on Treatment of ischemic central retinal vein occlusion with Method of Syndrome Differentiation Combined with Western Medicine

摘　　要：[目的]观察辨证分型联合西药治疗缺血型视网膜中央静脉栓塞疗效。[方法]使用随机平行对照方法,将58例住院患者按病志号抽签方法随机分为两组。对照组29例消旋山莨菪碱,10mg/次,1次/d,球后注射;复方樟柳碱,2mL/次,1次/d,颞浅动脉旁注射(双侧眼);单唾液酸四己糖神经节苷脂钠,40mg/次,1次/d,40mg+250mL NaCL,静脉点滴。治疗组29例1气滞血瘀-血府逐瘀汤;2肝阳上亢-天麻钩藤饮;3痰瘀郁滞-二陈汤和桃红四物汤;4阴虚火旺-知柏地黄丸;1剂/d,水煎300mL,早中晚口服;西药治疗同对照组。连续治疗15d为1疗程。观测临床症状、眼底血管、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组治愈7例,显效14例,有效6例,无效2例,总有效率93.10%。对照组治愈3例,显效5例,有效11例,无效10例,总有效率65.51%。治疗组疗效优于对照组(P<0.05)。[结论]辨证分型联合西药治疗缺血型视网膜中央静脉栓塞,疗效满意,无副作用,值得推广。[ Objective ] To observe the effects of syndrome differentiation combined with western medicine in the treatment of ischemic central retinal vein occlusion. [ Method ] Make use of randomized and paralleled control study and randomly divide 58 hospitalized patients into 2 groups based on their No. The control group consists of 29 cases using Raceanisodamine, 10 mg/times, 1 /d, retrobulbar injection;compound anisodine, 2 ml/time, 1 / d, para temporal superficial artery injection （ bilateral eye ） ;single sialic acid ganglioside hexose sodium four, 40mg/time, 1/d, 40mg＋250mL NaCL, intravenous drip. The treatment group also consists of 29 cases using （1）qi stagnation and blood stasis - Xuefu Zhuyu Decoction; （2）excessive rising of liver - Tianma Gouteng Decoction; （3）the phlegm and blood stasis stagnation -erchen decoction and Taohong Siwu decoction. （4）The hyperactivity of fire due to yin deficiency - Zhibai Dihuang wan. 1 dose/d, decocted into 300mL with water, orally three times a day;the other group take western medicine for the treatment. Take the treatment continuously for 15dys as 1 course. Observe the clinical symptoms, fluorescence fundus angiography and adverse reaction. Then take the treatment continuously for 2 courses and judge the curative effects. [ Results ] In the treatment group, 7 cases are cured; 14 cases excellent; 6 cases effective; 2 cases invalid with 93.10% as the total effective rate. In the control group, 3 cases are cured; 5 cases excellent; 11 cases effective; 10 cases invalid with 65.51% as the total effective rate. Curative effects in the treatment group are better than that in the control group （ P 〈 0.05 ） . [ Conclusion ] Treatment of ischemic central retinal vein occlusion with method of syndrome differentiation combined with Western Medicine is satisfactory without side effects and is worth spreading.
