辨证分型联合西药治疗脑梗塞偏瘫随机平行对照研究

摘　　要：[目的]观察辨证分型联合西药治疗脑梗塞偏瘫疗效。[方法]使用随机平行对照方法,将44例住院患者按就诊顺序号简单随机分为两组。对照组22例西医治疗,维持生命体征及处理并发症。治疗组22例辨证分型:阴虚风动:活血通络、育阴熄风(芍药12g,玄参15g,桃仁12g,桔梗5g,红花9g,麦冬10g,远志12g,生地15g,巴戟天10g,五味子12g,甘草6g);气虚血瘀:化瘀通络、益气养血(黄芪、丹参各15g,桃仁、赤芍各12g,石菖蒲8g,红花9g,当归10g,川芎12g,地龙7g,甘草6g);痰热腑实:通腹泻热、涤痰开窍(丹参25g,厚朴15g,生地10g,瓜蒌15g,桃仁12g,红花9g,胆南星8g,桔梗5g,郁金12g,芒硝8g,甘草6g);1剂/d,水煎200mL,早晚口服;西药治疗同对照组。连续治疗14d为1疗程。观测临床症状、肢体功能、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈11例,显效7例,有效2例,无效2例,总有效率90.91%。对照组痊愈8例,显效5例,有效3例,无效6例,总有效率72.72%。治疗组疗效优于对照组(P<0.01)。[结论]辨证分型联合西药治疗脑梗塞偏瘫效果显著,值得推广。[ Objective ] To observe the differentiation and type combined western medicine curative effect on cerebral infarction hemiplegia. [ Methods ] Using random parallel control method, 44 cases of hospitalized patients into two groups according to random number table method. Control group 22 cases of western medicine treatment, maintain vital signs and deal with complications. Treatment group of 22 cases：Yin deficiency type pneumatic：blood know luo, yuk Yin extinguish wind （Shaoyao12g, Xuanshen15g, Taorenl2g, Jiegeng 5 g, Honghua 9 g, Maidong 10 g, Yuanzhil2 g, Shengdi 15 g, Bajitian 10 g, Wuweizi12 g, Gancao 6 g） ; Qi deficiency and blood stasis type：blood know luo, tomifying qi and blood （ Huangqi, Danshen each 15 g, Taoren, Chishao 12 g, Shichnagpu 8 g, Honghua 9 g, Dangguil0 g, Chuanqiong 12 g, Dilong 7 g, Gancao6 g ）; Phlegm heat relieving excess type：through diarrhea, polyester hot phlegm un-awakened （ Danshen25 g, Houpul5 g, Shengdil0 g, Gualou 15g, Taoren 12g, Honghua 9g, Dannanxing 8g, Jiegeng5g, Yujin 12g, Mangxiao8g, Gancao6g ） ; 1 / d, Fried 200 ml water, morning and evening oral;Western medicine treatment with the control group. Continuous treatment of 14d to 1 course of treatment. Observation of clinical symptoms and adverse reactions. Continuous treatment 2 period of treatment, judge curative effect. [ Results ] in treatment group recovered 11 cases, 7 cases were markedly effective, effective in 2 cases, 2 had no effect, the total effective rate was 90.91%. Control group 8 cases cured, 5 cases were markedly improved, effective in 3, 6 no effect, the total effective rate was 72.72%. The treatment group curative effect is better than that of control group （ P〈0.01 ） . [ Conclusion ] Combined western medicine treatment of cerebral infarction hemiplegia effect is remarkable, is worth promoting.
