小儿手术中骶管阻滞复合小剂量氯胺酮-芬太尼麻醉的临床分析

摘　　要：目的探讨骶管阻滞复合小剂量氯胺酮-芬太尼麻醉在小儿手术中的应用效果。方法选取我院收治的88例需要外科手术的小儿患者临床资料进行回顾分析。结果两组患儿手术前生命体征(SBP、DBP、RR、HR、Sp O2)比较差异无统计学意义(P>0.05),手术结束后对照组患儿SBP、DBP、HR明显增高(P<0.05),两组患儿RR、Sp O2比较差异无统计学意义(P>0.05);观察组患儿麻醉优良率为95.5%;对照组患儿麻醉优良率为81.8%(P<0.05);观察组患儿氯胺酮用量为(42.1±10.9)mg,清醒时间为(15.4±6.8)min,1例发生不良反应;明显优于对照组患儿(P<0.05)。结论小儿手术采用骶管阻滞复合小剂量氯胺酮-芬太尼麻醉效果好,不良反应少,安全有效。Objective To study the application effects of sacral block with small-dose of Ketamine-Fentanyl anesthe- sia in pediatric operation.Methods The clinical data of 88 pediatric patients hospitalized to undergo surgical operation were analyzed retrospectively.Results The differences of vital signs such as SBP, DBP, RR, HR, and SpO2 in both groups before operation had no statistical significance（P〉0.05）.Afler the operation, SBP, DBP, and HR of the patients in the control group significantly increased（P 〈 0.05）, while the differences of RR and SpO2 in both groups had no statistical significance （P 〉 0.05）. In the observation group, the anesthesia excellent rate was 95.5 %, while it was 81.8 % in the control group（P 〈 0.05）.In the observation group,the Ketamine dosage was （42.1 ± 10.9）mg and the sober time was（15.4 ± 6.8）min with 1 case of adverse reactions,which was significantly better than the control group（P 〈 0.05）. Conclusion Sacral block with small-dose of Ketamine-Fentanyl anesthesia in pediatric operation has good effects with safety and low adverse reaction.
