不同手术方法治疗脊髓型颈椎病的疗效对比观察

摘　　要：目的：分析、对比不同手术方治疗脊髓型颈椎病的疗效。方法：选取2013年6月1日至2014年6月1日于我院接受治疗的90例脊髓型颈椎病患者作为研究对象，依照患者病变类型分为3纽：治疗1纽（30例）、治疗2组（30例）、治疗3组（30例），治疗1组病变类型为单个颈椎间盘突出，经颈前入路颈椎间盘切除＋椎体间植骨融合内固定术进行治疗；治疗2组病变类型为连续两个颈椎间盘突出，经颈前路椎体次全切除＋椎体间钛网支撑＋锁定钛板内固定术进行治疗；治疗3组病变类型为多个颈椎间盘突出，经颈椎后路单开门、椎管扩大成形＋侧块钉棒系统内固定术进行治疗。结果：治疗1组与治疗2组、治疗3组治疗优良率相当，无显著差异，P〉0．05，无统计学意义。结论：针对脊髓型颈椎病患者的病变类型，选择适当的手术进行治疗，才可确保治疗效果的有效性，且不同病变类型的治疗方法不可互相替代运用。Objective ： To study the effect of different operation therapy in the treatment ofpathologlcal types of cervical spondylotie myelopathy. Method： During 6/1 / 2013 to 6/1/2014, 90 cases of patients with cervical spondylotic myelopathy were selected, they were divided into three groups according to the lesion type： treatment group 1 （30 cases）, treatment group 2 （30 cases）, treatment group 3 （30 eases）, treatment group l single cervical disc hemlation, accepted after cervical anterior approach for cervical discectomy ＋ interbody fusion andinternal fixation ; treatment group 2 lesions type was two cervical intervertebral disc hemlation, accepted anterior cervical vertebral body subtotal resection ＋ interbody titanium mesh ＋ locking titanium plate internal fixation; treatment group 3 lesions type was multiple cervical intervertebral disc herniation, accepted after cervical posterior single open - door, cervical vertebralcanal enlarging plastic operation ＋ internal fixation of lateral mass screw - rod system. Results： Treat- ment 1 grOup and treatment group 2 and 3 treatment group had a similar highrate, had no significant difference, P 〉 0.05, and had no statistical significance. Conclusion ： According to the pathological types of cervical spondylotic myelopathy, select the appropriate operation, it can ensure effective treatment effect.
