丁苯酞对老年血管性痴呆患者认知功能的影响及作用机制

摘　　要：目的观察丁苯酞对老年血管性痴呆(VD)患者认知功能的影响并探讨其可能的作用机制。方法将84例VD患者随机分为观察组和对照组,每组42例。两组均给予常规内科治疗,观察组给予丁苯酞软胶囊口服,0.2 g/次,3次/日;对照组给予甲磺酸双氢麦角毒碱片口服,1 mg/次,3次/日。两组疗程均为12周。分别于治疗前后采用简易精神状态检查表(MMSE)、临床痴呆程度量表(CDR)和日常生活能力量表(ADL)对患者疗效进行评估。测定患者治疗前后血清超氧化物歧化酶(SOD)活性及丙二醛(MDA)含量。结果观察组治疗后MMSE,CDR及ADL评分均较治疗前显著改善,与对照组治疗后相比差异显著(P<0.05),治疗总有效率明显高于对照组(P<0.05);观察组治疗后血清SOD活性明显增高,MDA含量明显下降,与对照组治疗后相比差异显著(P<0.05)。结论丁苯酞可显著改善VD患者的临床症状,疗效确切,具有良好的安全性,其作用机制可能与其抗氧化损伤有关。Objective To observe the clinical efficacy of butylphthalide in the treatment of elderly vascular dementia（ VD） and to investigate its possible mechanism. Methods 84 cases of elderly VD were randomly divided into the observation group and the control group with 42 cases in each group. The two groups were given the conventional medical treatment. The observation group received oral butylphthalide 0. 2 g each time,3 times a day,and the control group received oral Co- Dergocrine Mesilate Tablet 1 mg each time and3 times a day. The treatmen course in the two groups was 12 weeks. The mini- mental state examination（ MMSE）, clinical dementia rating（ CDR） and activities of daily living（ ADL） scale were adopted for the assessment of curative effect. The serum SOD activity and the MDA content were measured before and after treatment. Results The MMSE, CDR and ADL scores in the observation group were significantly improved after treatment and significantly better than those in the control group（ P〈0. 05）. The total effective rate in the observation group was significantly higher than that in the control group（ P〈0. 05）; the serum SOD activity after treatment in the observation group was significantly increased and the MDA content was significantly decreased, which showed the statistically significantly difference compared with the control group（ P〈0. 05）. Conclusion Butylphthalide can obviously improve the clinical symptoms of VD and has definite curative effect and better safety,its action mechanism may be related with its anti- oxidative damage.
