摘 要:目的探讨新生儿高胆红素血症患儿心肌酶谱、肌钙蛋白I(cTnI)与光抑素C(Cys C)变化的临床意义。方法选取新生儿高胆红素血症52例为观察组,健康足月新生儿46例为对照组,黄疸治疗前后进行肝肾功能、心肌酶谱、cTnI、Cys C检测及心电图监测。结果观察组心肌酶谱、cTnI、Cys C水平明显增高;观察组患儿治疗前后心肌酶谱、cTnI、Cys C比较,差异有统计学意义(P<0.01),观察组患儿治疗后,心肌酶谱、cTnI、Cys C与对照组比较,差异无统计学意义(P>0.05)。结论新生儿高胆红素血症可以导致心肌及肾脏损害,经积极退黄、保心、补液等对症支持治疗后心肌酶谱、cTnI、Cys C明显降低,心肌及肾损害为可逆性改变,预后良好。Objective To investigate the clinical significance of myocardial enzymogram,cardiac troponin I(cTnI)and cystatin C(Cys C)changes in neonatal hyperbilirubinemia.Methods 52 neonates with hyperbilirubinemia were collected as the observation group and 46 normal term neonates as control group.The liver and kidney function,myocardial enzymogram,cTnI,Cys C as well as electrocardiogram were detected before and after jaundice therapy.Results The myocardial enzymogram,cTnI and Cys C levels in the observation group were significantly increased.And all the indexes levels were statistically different between before and after treatment(P〈0.01).While,,The myocardial enzymogram,cTnI and Cys C levels after therapy in the observation group had no statistically significant difference compared with the control group(P〉0.05).Conclusion Neonatal hyperbilirubinemia could lead to myocardial and renal damage.The myocardial enzymogram,cTnI and Cys C levels can be significantly decreased by the symptomatic and support treatment of removing jaundice,heart protection and liquid infusion.Besides,myocardial and renal damage is reversible change with good prognosis.
